Walking rounds with offgoing nurse to receive updates on each resident Receive, transcribe, and initiate orders from physicians Administer medications and complete any treatment orders as ordered by physicians Ensure all medications are ordered through the Pharmacy in a timely manner Collaborate with the MDS/Care Plan Team to develop and maintain up to date plan of care for residents Complete required documentation in the resident's chart Responsible to contact Physician and family for any change of condition with resident Walking round with physicians when they are in the building Supervise Certified Nursing Assistants on the unit Ensure tasks are complete prior to the end of shift Walking rounds with oncoming nurse to give update on resident status Other Requirements: Clear/Active Florida License or Compact License Pass a Level II Background Check Must be able to pass a drug screen and be medically cleared Effective October 13, 2024
Assist residents with Activities of Daily Living such as: Bathing Grooming Toileting Dressing Transfers Supervision 1 person assist 2 person assist 2 person assist with mechanical lift Meals Independent Set Up Supervision Limited Assistance Extensive Assistance Total Assistance Turning and repositioning Ensure residents have all Medical Equipment and supplies they need Monitor vitals and patient behavior and report to nurse on duty Complete restorative or Functional Maintenance Exercises as ordered Ensure documentation is accurate and complete each shift Participate with MDS/Care Plan Coordinator to develop resident plan of care Complete walking rounds with oncoming and offgoing CNAs to be up to date on resident care Other Requirements: Clear/Active Florida License Pass a Level II Background Check Must be able to pass a drug screen and be medically cleared COVID 19 Boostered $16/hr-17/hr $2 Weekend differential $2 Night shift differential
Must be proficient in MDS 3.0/PDPM TASKS & RESPONSIBILITIES: MDS COORDINATORS, ALSO KNOWN AS RESIDENT ASSESSMENT COORDINATORS & NURSE ASSESSMENT COORDINATORS, ASSESS & EVALUATE THE QUALITY OF EMOTIONAL, MENTAL, & PHYSICAL CARE BEING GIVEN TO LONG TERM RESIDENTS THE MDS COORDINATOR IS RESPONSIBLE FOR A BASELINE CAREPLAN WHICH IS IMPLEMENTED WITHIN 48 HOURS OF ADMISSION. THE BASELINE IS FURTHER EXPANDED AFTER THE COMPLETION OF THE COMPREHENSIVE ASSESSMENT & A SHORT-TERM & LONG-TERM CARE PLAN WITH GOALS FOR IMPROVEMENT IS DEVELOPED. MDS COORDINATORS SCHEDULE & CONDUCT MEETINGS WITH THE INTERDISCLIPINARY TEAM, CARETAKERS, & FAMILIES TO DISCUSS THESE GOALS & PLANS OF ACTION. MDS COORDINATORS REVIEW THE MEDICAL RECORDS FOR ACCURACY & ARE RESPONSIBLE THE ACCURACY OF CODING ON THE MDS WHICH IS ESSENTIAL FOR THE FACILITY PROPER REIMBURSEMENT. ADDITIONAL RESPONSIBILITIES INCLUDE BOWEL & BLADDER ASSESSMENTS & ICD 10 CODING ACCURACY. A WEEKLY SCHEDULE MUST BE PREPARED TO CAPTURE THE APPROPRIATE ASSESSMENT DUE FOR EACH MEDICARE & HMO ASSESSMENT. THE SCHEDULE FOR OPENING & CLOSING ASSESSMENTS ARE GOVERNMENT DIRECTED. IT IS MY RESPONSIBILITY TO PREPARE THE SCHEDULE, OPEN, & CLOSE THE ASSESSMENTS. MY DUTIES ALSO INCLUDE PREPARING & ELECTRONICALLY TRANSMITTING REPORTS TO THE NATIONAL MEDICARE & MEDICAID DATABASE.
