Home Health Aide (HHA)

Responsibilities Works under the supervision of the Director of Home Care or Hospice and registered nursing staff by assisting in the care of patients by bathing, dressing, feeding, transporting, and providing for the basic needs of the patient in the home environment. Assists caregivers in other home duties as requested within the scope of the job duties. Provides assistance in a supportive manner to caregivers as they are in the learning process of caring for the patient at home. Monitors vital signs and other patient activities, reports changes to the registered nurse. Functions at a level to maintain a high quality of care. Qualifications Work requires the ability to read well enough to follow specific and detailed nursing procedures, the ability to write legibly, use good English grammar, spell accurately and communicate effectively with a variety of people. This level of education is normally acquired through completion of a high school education. Completion of a home health aide course (75 hours in length), or one to two years recent home health, hospice, or hospital experience, or a certified nursing assistant certificate with six months experience, or student nurse experience and successful completion of course in fundamental in nursing in an RN program. Must have own car and adequate insurance coverage. Must have a current Indiana driver’s license. Must pass criminal record check prior to employment. Successful completion of ISHHA standardized test prior to employment. Must be free of infectious disease. Medical Assistant - Triage

Primary Care Advanced Practice Provider (Float)

Responsibilities Conduct comprehensive patient assessments, including history taking and physical examinations across assigned clinic sites. Diagnosis and manage acute, chronic, and preventative health conditions in alignment with evidence based guidelines. Develop and implement patient care plans, including ordering and interpreting diagnostic tests, prescribing medications and initiating referrals as deemed appropriate. Maintain accurate, timely, and thorough documentation in the electronic medical record (Cerner).
