OVERVIEW: Assist the Director of Nursing and Associate Director of Nursing in planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of the Nursing Department. Maintains quality clinical care for residents/patients in accordance with current rules, regulations, and guidelines that govern the Facility. RESPONSIBILITIES: Supervises the clinical care planning and delivery processes to ensure the coordination of quality patient care. Reviews and updates the care plan accordingly Assesses residents/clients upon admission and periodically as conditions change Performs rounds to evaluate patient condition Provides or assists with the delivery of direct patient care when indicated, including activities of daily living, meal assistance, medication and treatment administration, and leisure activities when indicated Obtains and implements Physician Orders, checking for accuracy and completeness Communicates between shifts regarding changes in resident/patient conditions Ensures a safe and secure environment for patients/residents, staff and visitors Provides clinical assessment to MD/NPs on call for quality outcomes delivered to patients Plans, implements and evaluates the clinical competencies and on-going educational/orientation needs of the staff. Participate in Facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing Services Participate in developing, implementing, and periodically updating the resident’s written discharge plan Ensure that a sufficient number of nursing staff is available for each shift to ensure that quality care is maintained Make daily rounds of the Nursing Services Department to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Report findings to the Director of Nursing Services Review complaints and grievances made or filed by department personnel. Make appropriate reports to the Director of Nursing Services, as required or as may be necessary Maintain nursing supplies inventory and other responsibilities as required Assist in the Quality Assurance and Performance Improvement (QAPI) process QUALIFICATIONS: EDUCATION / EXPERIENCE: Current registration and license to practice as a Licensed Practical Nurse in Floridan a hospital, nursing home or other healthcare setting and a minimum 1-year experience as a nurse supervisor CPR/BLS certified Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care centers. The ability to read, write, speak and understand the English language required, knowledge of other language helpful SUPERVISORY RESPONSIBILITIES: Supervises the nursing personnel and the day-to-day nursing activities of the facility during an assigned tour of duty WORK ENVIRONMENT: This job operates in a health care setting. Interact with residents in a manner that displays warmth and promotes a caring environment Fully understand all aspects of residents’ rights, including the right to be free of restraints and free of abuse Responsible for promptly reporting to the charge nurse or administrative staff incidents or evidence of resident abuse or violation of residents’ rights Assist in maintaining a safe, neat, and clean environment; report environmental deficiencies to the charge nurse such as lighting or equipment problems Protect the personal belongings of each resident including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, memorabilia, etc. Promptly report missing items according to established policy and participate in efforts to locate missing items Observe residents for changes in condition or behavior and promptly report these changes to appropriate licensed nursing personnel Provide care that maintains each resident’s skin integrity to prevent pressure ulcers, skin tears, and other damage by changing incontinent residents, turning, repositioning immobile residents, and applying moisturizers to fragile skin, etc. Answer residents’ call bells promptly and courteously Perform various tasks assigned by the charge nurse including vital signs, residents’ weights, applying creams/ointments, collecting specimens, etc. Assist with orienting residents and their families to the nursing home upon admission and to the unit when transfers occur Assist and escort residents for appointments such as beauty shop, activities, church services, etc. Complete certified nursing assistant records while using proper coding of activities of Daily Living (ADLs) documenting care provided or other information in keeping with department policies Follow prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials Support an atmosphere which promotes a homelike environment Assist in the Quality Assurance and Performance Improvement (QAPI) process QUALIFICATIONS: EDUCATION / EXPERIENCE: A high school diploma or equivalent; successful completion of a state-approved Nursing Assistant Certification course or other education which meets state certification requirements Must hold current Nursing Assistant Certifications in the state in which they are practicing Must be knowledgeable of resident rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all residents in a safe, secure environment. OVERVIEW: Responsible for the overall operation and optimal quality of care for the residents on the assigned unit. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and the Nursing Policies and Procedures. As an LPN you will be responsible for providing residents assistance with ADLs and in the administration of their medications while overseeing staff. Adhering to the instructions of Nurse Management, you will perform your duties in accordance with all company policies and procedures, while providing resident care services with kindness and compassion to the residents. RESPONSIBILITIES: Holds daily shift report with nursing assistants assigned to the unit prior to the start of the shift. Reviews written daily assignments for each resident on assigned unit. Assures assignments are completed before the end of the shift Makes rounds on assigned residents at least once every two hours Administers medication and treatments according to the physician's orders. Assumes responsibility for ordering medications Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and update these plans on a monthly basis or more often if the resident's condition changes Assists the attending physician with rounds and special procedures as needed Transcribes, documents and implements physician's orders Contacts the resident's physician and family to notify them of a change in the resident's condition or accident/incident; also notifies the supervisor covering Assumes independent responsibility for yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps taken to improve their performance.
