Job Description The Nurse Navigator provides assistance in the improvement of the quality/delivery of patient care throughout the continuum of care. He/she works in collaboration with the program directors, data analysts, department leaders, staff and patients to develop and support the performance improvement initiatives. The Navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes for each patient including support for interdisciplinary and cross facility collaboration. In addition, he/she identifies opportunities to streamline care practice, using LEAN methodology, through development of evidence-based guidelines and protocols. Job Description The Registered Nurse who works in the school setting is required to work with a high level of autonomy, as they are the only medical professional on campus. School nurses (RN) provide health education to staff and students, performs complex health procedures (PEG tube care, Emergency Trach Care, etc.) and delegates non-complex procedures to Medical Assistants (MA), Trained Unlicensed Personnel, Paraprofessionals (TUP), Unlicensed Diabetic Care Assistants (UDCA), and School Health Assistants (SHA). The school nurse (RN) is responsible for administering, counting, and storing medications for students. The school nurse (RN) makes supervisory rounds for the School Nurse, Licensed Practical Nurse (LPNs), MAs and SHAs, and completes the environmental survey at assigned campuses. Additionally, the school nurse (RN) performs all other health related tasks as necessary. The school nurse (RN) is responsible for training staff on how to use emergency medications such as EpiPens, ensures that the emergency equipment (i.e., AEDs) are in working condition. #CB Responsibilities Major Roles Assess, plan, implement and evaluate nursing care to students with acute illness and injuries within Health Centers in Schools nursing guidelines.
