Supervise LPNs, LVNS and CNAs, providing them support in order that they may provide total nursing care for residents. ESSENTIAL FUNCTIONS OF POSITION: • Utilize the nursing process to assess resident care plans, implement interventions, and evaluate outcomes related to nursing care. • Interview applicants, recommend hires; participate in counseling to extent permitted by law. • Initiate and lead team conferences to develop individualized nursing care plans; assess and document resident’s condition and nursing needs; assign team members who have the capabilities and qualifications to meet resident’s needs. • Implement physicians’ orders; administer medications, start IVs, perform treatments, procedures and special tests and document treatment as required by company policy and local/state/federal rules and regulations. • Ensure supplies are used economically and equipment is clean and maintained in a safe manner. • Coordinate nursing care of residents scheduled for therapy or procedures by other departments. • Report and record observations and reactions regarding residents. • Assist or institute emergency measures for sudden adverse developments. • Perform additional duties as assigned. EDUCATION/REQUIREMENTS: Must be skilled in directing, motivating and training staff and able to work as a member of a team; able to communicate effectively with residents, family members and all levels of the organization and maintain confidentiality. Should be a graduate of an accredited RN School of Nursing with valid RN state license, and a minimum of 2 years nursing experience; or equivalent combination of education and experience

POSITION PURPOSE: Organizes and directs assigned unit duties to provide quality nursing care to residents. ESSENTIAL FUNCTIONS OF POSITION: Responsible during the shift for the total nursing care of residents in the ass ig ned unit. Assumes responsibility for compliance with Federal, State, Local, and company regulations. Explains procedures and treatments and care plan to resident. Administers prescribed medications and treatments in accordance with approved nursing techniques. Transcribes physician's orders. Charts progress notes in an informative, factual manner that reflects the care administered as well as the resident's response to care. Receive s/g ives report from/to nurse he/she is relieving / coming on and records sufficient information and implements appropriate follow-up action as necessary Follows established procedures for charting and reporting all reports of incident/accidents for residents and employees. Conducts frequent rounds to evaluate effectiveness of care delivery. Prepares equipment and aids Physician during treatment and examination of resident. Maintains awareness of comfort and safety needs of resident. Observes resident, records significant conditions and reactions, and notifies supervisor or Physician of resident's condition and reaction to drugs, treatments, and significant incidents. Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess condition of resident. Responds to emergency situations based upon nursing standards, policies, procedures, and protocol. Assists in completion of the care plans (acute and long term) and is involved in the care conference as deemed necessary. Participates, reports to DON / Administrator and conducts (as appropriate) invest ig ations related to any alleged abuse. Ensures that documentation is reflective of the care plan and that the approaches on the care plan are being followed. Assures that ass ig ned CNA's are aware of the resident care plan, and ensure that the nursing assistant refers to the care plan prior to administering care. Monitors that CNA's documentation is completed daily. Takes report form CNA's to facilitate accurate documentation in resident medical record. Follows company policy and procedure in the event a restraint must be utilized. Coordinates residents' rehabilitation program with therapists to ensure 24 hour rehabilitation needs are met. Identifies residents at risk for nutritional problems and implements preventative measures. Records new/changed diet orders. Forwards information to the Dietary Department. Makes beds, bathes and feeds resident, when necessary. Assures documentation is accurately completed for admission, discharge and transfer. Supervises and evaluates implementation of the “Resident's R i ghts”. Participates in department or unit quality improvement activities. Follows facility procedure requesting supplies and cha rg es for items ordered thro ug h Central Supply. Participates and attends periodic in-services and staff meetings. Perform additional duties as assigned. EDUCATION / REQUIREMENTS: Current State of LVN/LPN with a minimum of 2 years related experience; or equivalent combination of education and experience. Good standing in the state registry is required.
POSITION PURPOSE: Ensures that resident's personal care needs are being met in accordance with Residents' wishes and Care Plan Objectives. Essential Functions of Position: Answers signal lights, bells, or intercom system to determine residents' needs and ensures call lights are within reach of resident. Ensures that residents' who are unable to call for help are checked frequently. Gives direct resident care such as bathing, dressing, and grooming of residents. Explains procedures to residents. Assists residents as necessary in their activities of daily living. Assists residents with bowel and bladder functions. Assists residents in preparation for and transporting to appointments, activities and social programs. Assists with lifting, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., in keeping with specific resident safety needs. Provides incontinent care as needed. Dresses resident appropriate to the current weather. Notifies Supervisor if resident needs clothing. Changes bed linens. Handles linens according to infection control guidelines. Feeds residents requiring help and records meal intake. Delivers and documents all snacks and supplements. Offers fluids to residents frequently to ensure that residents are hydrated where designated by care plan. Reviews and implements daily nursing needs as outlined by resident Care Plan or nurse managers. Transports residents or assists residents with mobility needs. Turns and repositions bedfast residents, alone or with assistance. Takes and records temperatures, blood pressure, pulse rates, respiration rates, height, weight, and food and fluid intake and output, as directed. Monitors resident's activities and reports any irregularities of resident performance to nurse managers. Participates in and receives nursing report upon reporting and departing to / from work. Reports all changes in residents' conditions to supervisor. Cleans instruments and equipment and returns to proper area after use. Maintains cleanliness of residents units, central bathing area, treatment and supply rooms, workrooms, and linen closets. Provides post-mortem care as instructed. Accurately records all care as directed. Participates and attends periodic in-services and staff meetings. Understands and adheres to the guidelines of "Residents Rights". Perform additional duties as assigned. EDUCATION / REQUIREMENTS: Ability to communicate with residents and nursing staff. Ability to speak, read, and write English. Prior experience as LTC nursing assistant preferred; or equivalent combination of education and experience. ESSENTIAL FUNCTIONS OF POSITION: • Prepare medications for resident administration as specified in the physician order, verifying medication, dose, route, time, and reason for administration. • Safely administering resident medication by verifying correct resident, obtaining any required vital signs pre-administration, assisting with resident repositioning as needed, and administering at a pace comfortable to the resident. • Document medication administration in the medical record. • Maintain medication cart in a clean and efficient manner. • Ensure timely medication reorders from pharmacy as indicated. • Label/date opened medication bottles/vials/boxes as indicated. • Perform additional duties as assigned, which may include State Registered Nurse Aide (CNA) duties for which the KMA is qualified. This may include, but is not limited to, assists with resident transfers and positioning, ambulation, activities of daily living including personal care, or feeding assistance.
