This diverse 120-bed community is located in Madison, Ohio, a little over 40 miles northeast of Cleveland, just off the coast of Lake Erie. A caring and compassionate multidisciplinary team is eager to welcome you.

Medicare- and Medicaid-certified, Cardinal Woods offers short-term rehab, long-term skilled nursing services, memory care, and respite care in a cozy setting that feels like home. To help residents and patients lead a purpose-oriented life, Cardinal Woods provides excursions to local parks and downtown Madison Village, and hosts a variety of in-house programs and activities.
