The Brook at High Falls Nursing and Rehabilitation Center is currently seeking a Full-Time and Part-Time LPN to provide direct nursing care to the residents and to supervise the day-to-day nursing activities performed by the certified nursing assistants (CNAs). Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility and as may be required by the Director of Nursing or Nurse Manager to ensure that the highest degree of quality care is maintained at all times. EDUCATION, EXPERIENCE AND QUALIFICATIONS: Must possess a current, unencumbered, active license to practice as an LPN in NYS. DUTIES AND RESPONSIBILITIES: • Maintain the safety to the residents at all times. • Administer medications as directed by the physician orders. • Change dressings as directed by the physician orders and report all changes is wounds to RN Nurse Manager or Supervisor. Assure physician orders are updated. Maintain the Daily 24-hour Report and submit to Supervisor as required. • Direct CNAs in the completion of their duties. • Assist CNAs to help meet the needs of the residents when available (i.e. toileting, call lights, etc.) • Maintain a cohesive understanding with families/caregivers of the resident. • Complete accident/incident reports as necessary and chart all reports involving residents. • Report all complaints and grievances made by the resident to the appropriate source. • Perform any other pertinent duties as assigned by my supervisor. • I understand that emergency conditions may require me to temporarily work shifts other than the one for which I have applied and been assigned and agree to such scheduling change or overtime when necessary to ensure proper operation of the Homes as directed by my supervisor. DUTIES AND RESPONSIBILITIES: • Assume responsibilities for and direct activities relating to health and nursing care of all residents. • May be responsible for assisting with administration of medication and treatments. • Accurate reporting and documentation of resident status and care needs information. • Assist resident's attending physician during visits/rounds as needed. • Report to physicians residents' condition changes, etc., as indicated. • Will schedule and obtain laboratory specimens as ordered by the physician or Medical Director. • Will assist with follow-through or may initiate precautionary measures in case of suspected infection in cooperation with the DON • Assign personnel and make staff adjustments according to needed coverage to meet resident needs. • Acquire replacement staff when an employee calls in; assist with scheduling as needed. •Responsible for supervision of nursing staff, including LPNs and CNAs • Maintain an open line of communication with the Director of Nursing, Medical Director, Nurse Managers, other departments and consulting agencies of residents' status and needs and staff and unit needs. Responsible for notifying residents' primary contact if residents' status changes. • Ensure that established policies and procedures including dress codes are followed by departmental personnel. • Perform any other pertinent duties as assigned by the DON. Duties & Responsibilities: The Nurse Practitioner will have the following responsibilities: In partnership with the clinical team, provide medical highest quality medical care for the practice’s patients. In partnership with the Medical Director and Management, assist and support clinical personnel (e.g., clinical nurses, allied health professionals, EMR, telemedicine operations). Provide consultations and perform diagnostic and therapeutic procedures in their clinical discipline within their practice scope, consulting when appropriate the supervising physicians for assistance with both diagnosis and treatment. Qualifications : Education: Nurse Practitioner Degree from an accredited U.S. school is required. Experience: At least 3 years of active practice required. DEA Certification required. Communication: Excellent verbal, written communication, and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner.
