Assist the MDS/Care Plan Coordinator with planning, scheduling, and revising MDS assessments, including implementing RAPs and triggers. Qualifications Registered Nurse (RN) with 1+ year of leadership experience as an ADON, DON, or similar role in long-term care. Experience with the geriatric population strongly preferred.
