Position Overview The RN Nurse Supervisor will primarily serve as the facility's Infection Preventionist. This Supervisor reports to the Director of Nursing and partners with the Medical Director, Medical Director for Antimicrobial Stewardship, the Quality Officer, and other stakeholders to develop a system of care that promotes sound and scientific infection prevention principles and practices.This individual is accountable for decreasing the incidence and transmission of infectious diseases between patients, staff, visitors and the community. Through strategic planning, leadership and consultation, you will lead and direct a robust team in the identification and implementation of infection prevention goals and objectives throughout the facility. Position Responsibilities and Qualifications In this key position, essential job duties and responsibilities include:Partners with the Medical Director for Epidemiology/Infectious Disease to develop, implement and evaluate annual infection prevention goals and action planPartners with facility leaders, physicians, local, state, and national agencies on activities related to infection preventionOversees the operations of the infection prevention and antibiotic stewardship programsAccountable for surveillance of healthcare acquired and community acquired infectionsManages goal setting process for system, unit and physician goals related to healthcare acquired infections and conditionsManages facility immunization programs Leads facility wide interdepartmental infection control program to anticipate, recognize, evaluate, mitigate and control workplace conditionsParticipates in the national collaboratives and external reporting to CDC NHSN system, and other post-acute care specific quality organizationsAuthority and responsibility for ensuring appropriate intervention and education occurs with staff, volunteers and medical staff when healthcare infection trends, outbreaks or non-compliance to infection control/OSHA are identified.Ensures that education and counseling on infection prevention is available and ongoing for all staff, volunteers, patients and patient representatives.Completes regular and ongoing infection control rounds, audits and surveillance throughout the facility in all departments Must be a member of the facility’s quality assessment and assurance committee and report to the committee on the IPCP on a regular basisParticipates in on call rotations as required by facility including assisting as a floor nurse if neededMaintains current knowledge of federal, state and local regulations and ensures that the facility leaders are informed of appropriate issues. Understands and complies with infection control, safety and OSHA procedures and regulations.

JOB SUMMARY: The purpose of this position is to assist the nurses in the providing of resident care primarily in the area of the daily living routine. QUALIFICATIONS: Must hold a current Certified Nurse's Assistant certificate in the State of Illinois. A high school diploma is preferred. Must have the ability and the willingness to work harmoniously with professional and non-professional personnel. Must be able to read, write, and speak English in an understandable manner. JOB REQUIREMENTS: Must be physically and mentally capable of performing routine, repetitive job duties. Show willingness to provide good nursing care. Be considerate, pleasant, and professional in the performance of his/her duties. Must have compassion, tolerance, and understanding for the residents of the facility. Knowledge of JCAHO, OBRA, IDPH, and HFS regulations as related to duties. MAIN DUTIES: Support the facility's philosophy of care and strive to achieve its goals and objectives. Be sensitive to residents' families and respond in an appropriate professional way as the situation requires. Carry out assignments for resident care including (but not limited to): a) bathing b) dressing c) grooming d) shaving e) feeding f) restorative nursing procedures g) retraining Keep residents' bed, dresser, bathroom, and general living area clean and tidy. Answer call lights promptly. Help check in all new residents and re-admissions, as well as assist in the transfer of residents to different rooms. Put resident's clothing away properly. Report any changes in resident's condition--e.g. eating habits, behavior, temperature, etc. to the charge nurse of the unit. Turn bedridden residents every two (2) hours or sooner as indicated. Wash, clean, and dry all incontinent residents. Assist the treatment nurse as assigned. Complete assignment report to charge nurse at the end of every shift. Be responsible for well-being and nursing care of all residents assigned to his/her unit while on duty. Take vital signs and monthly weights (with assistance) and record results in EMR. Document the following in the EMR for all assigned residents: a) vital signs b) weight c) meal monitoring d) bowl and bladder e) restorative records Detect and report situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Follow established safety precautions when performing tasks and when using equipment and supplies. Assure that equipment is cleaned and prepared for the next shift. Assure that work schedules are followed. Maintain the confidentiality of resident information and honor his/her personal and property rights. Attend nursing department, Care Plan, and other meetings as directed by the DON or ADON. Attend and participate in in-service educational classes and on-the-job training programs as scheduled or as directed. Turn in Days Off and payroll special request forms in a timely manner. Follow established fire, disaster, safety, infection control, and evacuation policies and procedures. Perform other related duties as directed by the DON, ADON, charge nurse, or the Administrator. FULL-TIME ACTIVITY AIDE Responsible for performing the daily activity calendar requirements set by the Activity Director. Required to support and follow the Activity Program’s mission, philosophy, policies and procedures and state and federal regulations. Will achieve and maintain the highest quality of life for each resident, with understanding of all psychosocial, medical, and activity needs/preferences. QUALIFICATIONS: •Hold a high school education or equivalent. •Have at least 1 year experience working in the healthcare field, volunteering or personal experience with the disabled population. •Current knowledge of Federal and State activity regulations and/or ability to understand and interpret. •Be able to read and write English. •May prefer bi-lingual abilities based off of individual facilities. •Must have a valid, IL driver’s license. •Must be punctual and time efficient, especially as it relates to following the activity calendar. •Possess the qualities of empathy, compassion, understanding, enthusiasm, personable, energetic, positive and humorous in order to effectively interact with residents, families, and staff. •Must be able to relate and work with ill, disabled, elderly, emotionally upset, and at times hostile people. •Must be able to communicate effectively with to the community, other health professionals, outside professional organizations, senior services, government agencies, and any other provider or professional in the community. •Understand and be able to manage emotional needs of yourself and the staff.
