Initially focused on heart failure and cardiac transitional management in partnership with acute care hospitals, this role will evolve to support other complex medical and specialty programs as they expand across Apple Rehab. The Coordinator provides advanced clinical oversight, education, and communication between residents, families, facility staff, and hospital partners to prevent avoidable readmissions and promote recovery and stability.
