JOB SUMMARY Responsible to provide direct nursing care to the customer, and to supervise the day-to- day nursing activities performed by the nursing assistants. Such supervision must be in accordance with current Federal, State, and local standards, guidelines and regulations, facility policies. The objective is to ensure the highest degree of quality care is maintained at all times. QUALIFICATIONS Must possess a current, unencumbered license to practice as a RN or LPN in the State of Illinois and/or Wisconsin. Must possess current CPR certificate or become certified within two weeks of employment. Must be knowledgeable of nursing and medical practices and procedures. Demonstrates interest and/or knowledge in care of the aged. Must be able to read, write, and speak the English language in an understandable manner. ESSENTIAL FUNCTIONS Accepts and supports as directed by appropriate authority. This may require different floor, customers, workload, and multiple tasks as assigned. Plan, develop, organize, implement, evaluate, and direct the Ladies and Gentlemen of Alden for a respective unit. Assume all Nursing procedures and protocols are followed in accordance with established policies. Directly supervise and instruct the Ladies and Gentlemen of Alden on a number of topics to promote job knowledge. Ensure all the Ladies and Gentlemen of Alden follow established departmental policies and procedures, including appropriate dress code. Assist in developing a written plan of care for each customer that identifies the problems/needs of the customer. Attends and actively participates in customer care plan conference as directed. Report all concerns, complaints made by the customer or their family to the Administrator. Counsel/discipline nursing personnel as requested or as necessary. Make daily rounds to ensure nursing personnel are performing required duties, and to ensure appropriate procedures are being followed. Make physical rounds to all customers daily. Make rounds with physicians as necessary. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department. Attend and participate in continuing education programs. Recommend to the Director of Nursing the equipment and supply needs of the department. Place orders for medications and treatments as necessary following established budgetary guidelines. Utilize PCC 24-hour summary concerning recommendations/concerns related to customer care needs. Fill out complete accident/incident reports. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned. Will achieve and maintain the highest quality of life for each resident, with understanding of all psychosocial, medical, and activity needs/preferences. QUALIFICATIONS Hold a high school education or equivalent. Have at least 1 year experience working in the healthcare field, volunteering or personal experience with the disabled population. Current knowledge of Federal and State activity regulations and/or ability to understand and interpret. Be able to read and write English. May prefer bi-lingual abilities based off of individual facilities. Must have a valid, IL driver’s license. Must be punctual and time efficient, especially as it relates to following the activity calendar. Possess the qualities of empathy, compassion, understanding, enthusiasm, personable, energetic, positive, and humorous in order to effectively interact with residents, families, and staff. Activities shall be daily and shall reflect the schedules, choices, and rights of the residents. Be able to understand communication needs of residents, such as language services, communication boards, etc. Per direction of the Activity Director, conduct, oversee, and complete initial and all on-going assessments, including but not limited to social, medical, cognitive, communication abilities, spiritual needs, physical, neuro-psychological, behavioral, amongst others as it relates to individual activity/leisure. Document and complete all required documentation per Activity Director. Maintain the following records: Resident Daily Participation, Resident Progress notes, Leisure Interest Survey, and Activity Planning sheets to assist in scheduling and meeting resident needs. Assist to update activity boards through direction of the Activity Director, with a monthly theme that is creative, neat, and clean. Facilitate community involvement, such as inviting the community to events and programs. Assist in resident outings, which can include but not limited to shopping centers, movies, restaurants, community events, etc. that may also entail driving residents to and from the resident outing. Follow Activity staff schedules with a team approach and continuity of care. Work as team with staff and volunteers. Must maintain structure of activity schedule and conduct all activities on-time; May not cancel an activity without permission of Activity Director. Collaborates with CNAs to ensure compliance with established departmental policies and procedures, including appropriate dress code. Assist in developing a written plan of care for each customer that identifies the problems/needs of the customer. Attends and actively participates in customer care plan conference when indicated Resolves then processes a concern resolution form on all concerns, complaints made by the customer or their family to the Administrator. Under supervision of a clinical leader, meets with the CNA Ladies and Gentlemen of the nursing department, on a regularly scheduled basis to assist in identifying and correcting problem areas, and/or the improvement of services. Recruit and conducts preliminary interviews and make recommendations for hiring to the clinical leadership and Administrator. Final interviews and approval are completed by the clinical leadership or Administrator. Provides assistance with orientation, coaching and mentoring to ensure the success of CNAs in the performance of their duties. Makes recommendations for counseling to the clinical leadership. Provides feedback to clinical leadership regarding CNA daily and annual performance evaluation feedback. Make daily rounds to ensure that nursing personnel are performing required duties, and to ensure appropriate procedures are being followed. Make physical rounds of all customers daily. Plan, conduct and participate in In-service Education Programs. Assist in implementing and maintaining an effective orientation program that orients the new CNA Ladies and Gentlemen of Alden to the facility, his/her department, and its policies. Attend and participate in continuing education programs. Recommend to the Director of Nursing the equipment and supply needs of the department. Place orders for equipment and supplies as necessary following established budgetary guidelines and assists with the distribution of supplies to ensure that the CNAs have all necessary equipment and supplies. Assist in developing methods for coordinating nursing services with other customer services to ensure the continuity of the customers total care regimen. Monitor and perform administrative duties such as completing forms, reports, studies, charting, etc., as necessary. Monitor absenteeism to ensure that an adequate number of nursing care personnel are on duty at all times. Report problem areas to the Director. Assist and maintain general organization of nursing units. Provide the Director of Nursing with information relative to the nursing needs of the customers and the nursing service department’s ability to meet these needs. Assist with arranging appointments and transportation as needed. Assist support services in developing, implementing, and conducting in-service training programs that relate to the nursing services department. Ensure that all personnel involved in providing care to the customer is aware of the care plan and that care plans are used in administering daily care to the customer. Attend Department head meetings, Infection Control meetings, Quality Improvement meetings, etc., as directed. Maintains daily oversight of POC documentation compliance Perform other related duties as assigned.
