Job Description The Nurse Practitioner/Advanced Practice Practitioner is responsible for modeling the WellBe values and mission in care delivery for our patients. S/he is responsible for following the rules and regulations of state and federal regulatory agencies and any other certified agencies for their region. As an advanced practice provider at WellBe, you will play a vital role in the care for our patients in their home. This role is critical to our overall interdisciplinary team and focuses on providing compassionate care to the patients who have both chronic and acute illnesses and injuries while supporting their ability to stay in their own home. Our providers support a full patient panel and deliver longitudinal primary care, urgent care and acute medical care to a wide variety of patients. This role delivers primarily home-based setting care for their patients. The Nurse Practitioner role is a key member of the team that will be accountable for the care of their patient panel, maintaining accurate, timely and current patient records, and will as part of our longitudinal model will support assuring patients are followed regularly with initial, urgent, transition of care and follow up appointments as needed to care for their health needs. The successful candidate works as part of our broader interdisciplinary team to assist in delivering concierge-style, timely and high-quality care to every patient. Facilitates staff, patient, and family decision-making by providing educational resources. Performs primary care in home services, initial patient screenings/physical examinations, creates appropriate assessment/plan and completes documentation in a timely manner. Addresses any urgent care needs of the patients as needed, including transition of care visits. Collaborates with community team to complete a full patient care assessment and plan. Orders, interprets, and analyzes diagnostic tests and determine the need for follow-up. appointments and further treatment options per state supervisory agreement. Prescribes and manage medications and treatments as appropriate, including but not limited to as psychopharmacological agents per state supervisory agreement. Updates patient records and check records for accuracy at each patient appointment. Delivers patient care and completes regular patient reviews with supervising physician per state supervisory agreement. Delivers the highest standard of quality care while maintaining all company metrics pertaining to appropriate patient needs. Focuses clinical care delivery to support necessary items for documentation/coding, HEDIS measures, engagement of patients, and managing medical expenses. Serves as a key resource for patients in delivering ongoing care information, counseling and guidance for any patients with critical conditions. Utilizes all available resources to help care for patient needs and refers to internal/external resources as appropriate, such as WellBe behavioral health protocols. Collaborates with the interdisciplinary team and participate in regularly scheduled team meetings. Maintains required unencumbered, active state licensure(s) and national certification(s). Deliver palliative care, and when needed, support end of life care to patients and families. Maintains and accurately communicates updates for surgeon preference cards within the applicable service line. Anticipates the needs of the surgeon and nursing staff by passing instruments and supplies for surgical procedures, performing competently the role of the surgical technologist in the provision of surgical assistance during a variety of surgical procedures including but not limited to: orthopedics, gynecology, general, ENT, ophthalmology, urology, CV, spine, neurology, oral/maxillofacial, and minimally invasive procedures. Assists the circulating RN with sponge, instrument, needle, and other surgical counts. Role models behaviors and skills, through peer-to-peer accountability, towards improving department score for patient satisfaction. QUALITY/SAFETY ESSENTIAL FUNCTIONS Maintains highest standard of sterile technique during pre and post procedures. Prepares sterile dressings and/or immobilization devices. Cleans and prepares instruments for terminal sterilization, following surgical procedure. Assists other members of the team with terminal cleaning of the room. Assists with implementation and teaching of department-based safety initiatives and standards of practice. Problem-solves and manages multiple ongoing priorities with minimal supervision. Participates in debriefing and quality improvement practices to ensure quality patient care. Initiates corrective actions and improvement activities towards improving department and hospital quality and safety targets, reporting near misses, and collaborating with the interprofessional healthcare team. FINANCE ESSENTIAL FUNCTIONS Maintains efficient use of surgical supplies and minimizes opening of supplies until confirmation that will be used during surgery. Makes recommendations for cost containment during cases. Self-motivated to independently manage time effectively, minimizing incidental overtime, and prioritize daily tasks. May co-chair a unit-based Shared Governance Council. Acts as resource in surgical technology for the department on other hospital committees.
