Duties: Perform thorough in-person clinical evaluations for existing residents according to UAS standards, agency policies, and state laws. Create individualized service plans and determine eligibility for ongoing participation through detailed UAS assessments. Evaluate potential new residents by considering clinical, social, and environmental factors to establish appropriate levels of care ensuring safety and well-being. Gather and analyze clinical information from multiple sources to support patient care decisions based on clinical guidelines and professional judgment. Maintain clear and effective communication with residents, family members, healthcare providers, and other stakeholders to guarantee assessment accuracy. Apply a strong understanding of the challenges faced by vulnerable populations with chronic or complex bio-psychosocial needs. Ensure all assessments meet regulatory and organizational requirements.
