We are currently seeking a Licensed Practical Nurse (LPN) to join our caring team and contribute to the well-being of our residents. Position Overview: As a Licensed Practical Nurse (LPN) at Cedarvale Commons , you will work closely with a team of healthcare professionals to provide direct care to our residents. You will assist with patient assessments, administer medications, and monitor patient progress, while ensuring that each resident receives the highest level of care in a nurturing and respectful environment. Key Responsibilities: Assist in the assessment of patients' physical and mental health. Administer medications and treatments as directed by physicians or RN supervisor. Monitor patient progress and report any significant changes in condition. Assist in developing and updating care plans under the guidance of RNs and physicians. Provide compassionate care and comfort to residents and their families. Supervise and support nursing assistants (CNAs) in delivering quality care. Ensure compliance with all safety, infection control, and healthcare standards. Maintain accurate, up-to-date patient records in accordance with HIPAA guidelines. Assist with patient admissions, discharges, and transfers as needed. Qualifications: Current Licensed Practical Nurse (LPN) license in the state of Ohio. Previous experience in a skilled nursing facility or long-term care setting preferred, but not required. Strong knowledge of nursing practices, clinical procedures, and regulations. Excellent communication and interpersonal skills. Compassionate, patient-centered approach to care. Ability to work in a fast-paced, team-oriented environment.
