Job Opportunity: CNA Certified Nursing Assistant Starting Pay: Competitive rates starting at $20 per hour, based on experience Differentials Offered: Shift Differentials for 2nd and 3rd shifts Weekend Differential: $1.75 for hours worked on Saturday and Sunday Additional Shifts Differential: Extra $5.00/hour for any shifts worked outside scheduled hours Available Shifts: Part-Time 7a-3p Part-Time 3p-11p Full-Time & Part-Time 11p-7a Per Diem Responsibilities: Assists residents in daily bathing, grooming, dental care, bowel and bladder functions, dressing, walking and wheelchair use, positioning in and out of bed. C.N.A. Certified Nurse’s Assistant documents personal care given according to facility policies. Provides nursing functions as directed including perineal care, making beds, turning residents, measures and records temperature, weight, pulse and respiration. C.N.A. Certified Nurse’s Assistant observes and reports to supervisor changes in residents’ condition, including any skin breakdowns. Reports immediately all accidents or injuries. Records all necessary charting entries. Answers all call lights regardless of assignment. Responds to emergency situations calmly and efficiently. Qualifications: Must hold current Certified Nursing Assistant (CNA) certification in good standing. No history of patient/resident abuse as per State Registry. Proficient in reading, writing, and following oral and written instructions. Strong attention to detail, excellent follow-through, and ability to prioritize multiple tasks. Team player with good cooperation and interpersonal skills. Demonstrates patience, tact, enthusiasm, and a positive attitude towards residents and their support networks. Job Description $37-$48/ hr based on experience 16hrs/ week Please be advised - candidates without prior experience in MDS (minimum data set) will not be considered for this position. The LPN MDS Coordinator gathers information, assesses needs, establishes reasonable goals, provides interventions and incorporates within an organized, concise, functional care plan. Coordinates completion of comprehensive assessment by interdisciplinary team and includes recommendations in the written care plan for each resident. Each plan must identify all relevant issues for the care of the resident as well as the goals to be accomplished for each problem or need identified. The LPN MDS Coordinator works together with care planning team to implement final plans. Encourages the resident and his/her “responsible parties” to participate in the development and review of care plans. Care plans must focus on assisting residents to reach their highest practicable level of well being. The LPN MDS Coordinator ensures that all nursing personnel are aware of the care plan for each resident and that care plans are used in providing daily nursing services. Reviews nurses’ notes and monitors the resident to ensure the care plans are being followed and if each residents’ needs are being met. Assesses, reviews and revises care plans as required. Plans, schedules and conducts weekly care plan meetings for all residents according to OBRA and state requirements. Completes the MDS with utmost accuracy and insures highest level of reimbursement for facility. The ideal candidate will possess skills to maximize reimbursement as well as ensure Medicare compliance. The LPN MDS Coordinator complies with current CMS Mega Rule guidelines. Point Click Care experience a plus. Qualifications: Must have experience completing Minimum Data Sets (MDS) and resident care plans in the long term care sector. Must hold a current state LPN license and be a nurse in good standing. Must meet all applicable federal and state licensure requirements. Attention to detail, good follow through skills and ability to prioritize multiple tasks. Ability to instruct others.
