Nocturnal Advanced Practice Provider - Acute Care Services

The ideal candidate will be a Physician Assistant (PA) or Family Nurse Practitioner (FNP) with Acute Care Certification and a minimum of two years of inpatient clinical experience. Candidates must be flexible and willing to float within MD Anderson Cancer Center. This position follows core hours of 6:00 PM to 6:00 AM, with the orientation period scheduled during the overnight shift hours. Job description: The primary purpose of the Advanced Practice Provider is to provide patient care in the inpatient setting. The Advanced Practice Provider will work as a member of the APP Nocturnal Coverage Team to provide initial assessment and management of urgent issues at night related to internal medicine or critical care needs across the institution. The Advanced Practice Provider will facilitate communication and carry out orders from attending physicians related to patient care. Key Functions Provides competent patient care in the inpatient setting across the institution. Provides initial assessment and management for urgent internal medicine or critical care needs at night. Conducts comprehensive health assessments, identifies normal and abnormal health characteristics, initiates appropriate care and continuously evaluates care outcomes. Provides post-admission assessments for night time admissions. Completes sign-out communications and patient care logs as directed by the medical director and administrative director of the Nocturnal Coverage Team. May also perform delegated functions under the supervision of the physician to include differential diagnosis, use of prescriptive authority and treatment of a variety of human responses according to practice guidelines, protocols or standing orders. Uses judgment to collaborate with the healthcare team to manage client care throughout the continuum of care and in a variety of settings. Has primary responsibility for working with department faculty physicians to assist in covering the General Internal Medicine services. Clinical aspects include but, are not limited to the following: manage chronic medical illnesses, such as diabetes, HTN, COPD, assess and manage any common acute and episodic illnesses such as constipation, cough, diarrhea, n/v, suspected or new DVT, asymptomatic PE, URI symptoms. Assess and manage adverse events or side effects of patients receiving chemotherapy in the ATC, as well as management of the acutely ill hospitalized surgical patient. Perform and record histories, physical assessments and progress notes within 24 hours. For co- managed patients sign off and forward to the supervising physician for co-signature within 24 hours. Order routine tests appropriate to the services provided under such orders. Order patient medications and treatments within authorized limited prescriptive authority, consistent with institutional delegated prescriptive authority policies and procedures. Assist with clinic and hospital related paperwork (radiology and nuclear medicine forms, placing patients on clinical pathway, writing/calling in prescriptions) Retrieve and review labs, pathology results, radiology and other diagnostic reports Facilitate communication with supervising physician, consultants to determine appropriate disposition of patients seen in Consultative Medicine by reviewing laboratory and/or diagnostic results, and care coordination with various specialties. Independently initiate monthly assignments without supervision. Understand and respond to telephone conversations with physicians and other members of the health care team, both within and outside of the institution. Clearly write and communicate verbally, person-to-person, and over the phone. Attend Monthly Consultative Medicine Meetings. Attend Division of Internal Medicine Grand Rounds. Wound, Ostomy, and Continence (WOC) nurses provide recommendations for the treatment of complex wounds requiring advanced wound care products and active therapies. WOC nurses also provide consulting services for patients that require stoma site selection, as well as pre-operative and post-operative ostomy teaching. Ideal candidate must have a BSN degree with at least two years of registered nursing experience and graduation from an accredited Wound, Ostomy, and Continence Nursing Education Program.
