Full Time, Night shift (12 hrs) Provides prescribed medical treatment and personal care services to individuals receiving services under the direction of an RN or physician. ESSENTIAL JOB FUNCTIONS To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below: Provides basic care and medical treatment to individuals receiving services. Completes health assessments and provides input of health diagnosis to RN or physician for the development of nursing care plan. Follows-up on all physician orders and monitors and ensures follow through on individual’s medical/dental appointments. Monitors and records medications in a timely manner; acts as a liaison to pharmacy. Reviews medication error forms, analyzes errors and makes recommendations for procedural changes. Administers medication as required. Oversees and monitors staffs who administer medication. Conducts quality assurance checks on health care records. Provides emergency coverage and/or care for individuals receiving services. Maintains confidentiality, respects human rights and practices universal precautions in accordance with Mentor policy. Upholds policy and procedure development related to health care. Works with external agencies to coordinate services and ensure consistent treatment of clients. Administers, tracks, and maintains log for staff and individual’s testing. Provides training for staff including medication administration, universal precautions and infection control. Provides training for individuals receiving services such as self medication, prescribed treatments, sex education/STD prevention and smoking cessation. May be required to coordinate, organize and/or assist with household activities such as light housekeeping and meal preparation; transport individuals receiving services to planned and/or necessary activities, appointments. Participates in on call system as needed. Performs other related duties and activities as required. SUPERVISORY RESPONSIBILITIES None Required Minimum Knowledge and Skills required by the Job The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job: Education and Experience: Associates degree in nursing or related field or state certificate One year experience working with population served in a nursing capacity Certificates, Licenses, and Registrations: Current LPN/LVN licensure for state of residence Other certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training Blood Borne Pathogen Category: Job requires performance of duties that involve potential for exposure to blood, body fluids, or tissues. Tasks that do involve exposure are an expectation of employment. Other Skills and Abilities: N/A Other Requirements: Travel as needed Physical Requirements: Heavy work. Exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects. Certified Nursing Assistant Make a difference every day as a patient advocate providing quality of life-enhancing services to those we serve. As a Certified Nursing Assistant, you will perform hands-on nursing care with an emphasis on daily care needs and personal hygiene. Monitor the health conditions of individuals served and ensure quality services provided Provide assistance and/or supervision to individuals in the areas of daily living, personal care, and community/social integration Help maintain safe and quality supports for individuals served Initiate emergency procedures on a needed basis Use equipment correctly and safely Report any hazardous conditions, equipment, accidents, or incidents to supervisor Maintain current progress notes and any other appropriate documentation Assist with resident activity programs, including off-site outings and off-site appointments Assist with household activities Assist in keeping residence tidy and clean Assist with meal preparation and mealtimes Prepare rooms for admissions of new residents, obtaining equipment and supplies as needed Prepare residents for discharge Actively attend and participate in team meetings and training as required Maintain confidentiality and treat others with dignity and respect in all you do Qualifications: High School diploma or G.E.D. Completion of CNA certification program; Current CNA certification issued by state relevant to the position.
