Administers medications and treatments as prescribed. Performs patient assessments and monitors changes in patient condition. Communicates effectively with patients, families, and other healthcare team members to ensure quality care. Educates patients and families on care plans and safety measures. Documents all patient care activities in compliance with agency policies. Familiarity with Feeding Tubes, Assisted Breathing Devices (Ventilator) and Hoyer Lift. Job Requirements: Graduate of an accredited RN program Current RN license in Pennsylvania Minimum of one year experience in home healthcare or a similar clinical setting Familiarity with Feeding Tubes, Assisted breathing Devices (Ventilators) and Hoyer Lift CPR certification Strong communication and interpersonal skills Self-motivated with the ability to work independently Reliable transportation and valid Driver's License Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption. The Admissions RN is responsible for monitoring, documenting, and implementing treatment plans designed to enhance patient comfort, educate patients and families, and promote independence in self-care whenever feasible.
