The ideal candidate will be a road warrior, willing to travel throughout New Jersey to provide exceptional patient care. Responsibilities: Conduct comprehensive patient assessments and reassessments. Manage and track case progress, ensuring timely follow-ups and completing necessary assessments. Maintain thorough documentation and stay on top of assessment dates. Provide high-quality care and support to patients as needed. Communicate effectively with patients, families, and the homecare team. Requirements: Valid RN license in New Jersey. Reliable car and valid driver's license. Willingness to travel throughout New Jersey. Strong case management skills with the ability to manage multiple patients.
