Position Summary The PRN Nurse Practitioner provides services including assessments, clinical evaluations, medication management, patient education, and collaboration with the interdisciplinary team. This role supports home health patients with chronic conditions, acute changes, post-hospitalization needs, and ongoing follow-up care. The NP will serve as a virtual and in person clinical resource, ensuring timely and appropriate care while assisting in preventing unnecessary ER visits and hospital readmissions.
