As a Licensed Practical Nurse (LPN), you will be caring for a group of residents and assuming responsibility and accountability for them. Nursing is provided through assessment, implementation, and evaluation of the plan of care. Licensed Practical Nurse (LPN) Duties and Responsibilities: Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations. Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care. Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments. Charts nurse’s notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. Monitors and directs non-licensed ancillary services as deemed necessary to meet professional standards and maintain the highest practicable level of resident care. Communicate/report information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines. Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards. Ensures compliance with infection control standards. Immediately corrects/addresses identified instances of non-compliance. Follows and adheres to company policies and procedures regarding HIPAA (Health Insurance Portability and Accountability) compliance. All other duties, as assigned. Licensed Practical (LPN) Nurse Qualifications and Skills: Excellent written and verbal communication skills in English. Ability to multi-task and remain calm under stress. Analytical and problem-solving skills. Understands and follows infection control standards, policies and procedures. Must be able to adapt to a flexible work schedule and occasional overnight travel. Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other facility staff. Licensed Practical Nurse (LPN) Education and Experience: Nursing Degree or diploma from an accredited College or University. Possess a current license to practice in the State as a Licensed Practical Nurse. 1 year of experience preferred. Long-Term care experience preferred. Physical Requirements: Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required. The Assistant Director of Nursing RN supervises, analyzes, delegates and evaluates nursing services to maintain quality standards of care and advise medical staff, department heads, and Administrator in matters related to nursing services. Assistant Director of Nursing RN Duties and Responsibilities: Reviews daily work assignments to ensure they meet regulatory compliance and provide consistent staffing whenever possible. Assists DON in daily managing of the nursing staff, scheduling breaks, reassigning employees from one area to another area as facility needs dictate, and enforcing policies with the authority to issue Coaching and Corrective Action with HR/Administrator guidance. Responsible for recruitment, training, clinical orientation, supervision, scheduling, time cards, and personnel issues in the nursing department. Participates with the Medical Consultants and the Administrator in the selection of all potential new admissions to the facility and assess items needed in the facility to meet the new resident’s needs, admission, retention, and discharge of residents. Ensures necessary equipment, supplies, and staffing are available for the delivery of the prescribed resident/patient care. Manages the nursing department budget and provides Administrator with budgetary planning information related to equipment needs and staffing. Completes rounds of the facility at least daily to ensure compliance with all policies, procedures, and regulations. Collaborates with the DON and Administrator in the planning of staff development programs that will enhance staff knowledge in providing quality resident care. Works with regional personnel to continuously improve the quality of resident/patient care. Strategic plans or program development for improved outcomes to reduce unnecessary returns to acute care. Mentors nursing leaders and prepares for succession planning of role. Ensures that all elements of the facility’s Abuse & Neglect policies are maintained and enforced. Knows Resident Rights. Helps the residents/patients exercise and/or protect their rights. Follows and adheres to company policies and procedures implementing HIPAA requirements for the privacy and security of protected health information. Perform other duties as assigned. Assistant Director of Nursing RN Qualifications and Skills: Proven strong management skills, including delegating responsibility to subordinates, performance management, conflict management, and leading through others. Must have good financial and management skills. Must be able to adapt to a flexible work schedule and occasional overnight travel. Possess special interest in, and a positive attitude about, working with short and long-term care residents and the elderly. Be able to effectively communicate with a diverse range of audiences and settings: properly handle emotional situations, manage resources to accomplish resident care objectives, assess operational, staffing, and fiscal needs, analyze complex written documents, and work long and irregular hours. Ability to read, write, speak, and understand English. Demonstrates basic computer knowledge and ability with an aptitude to learn company applications as well as proficiency with Microsoft Office Suite (Excel, Work, Outlook, PowerPoint). Assistant Director of Nursing RN Education and Experience: Bachelor’s or Associate’s degree from an accredited School of Nursing. Current RN license with the state required. Prefer previous nursing and leadership experience of at least three years; ADON preferred. The Certified Nursing Assistant (CNA) helps patients/residents with comprehensive and personalized health care needs under the supervision of a nurse. The CNA assists patients/residents with activities of daily living to include bathing, dressing, eating, grooming, toileting and exercising. At this facility, this is a collectively bargained position under the union. Certified Nursing Assistant (CNA) Duties and Responsibilities: Under the direction of the Director of Nursing or CNA Supervisor, the CNAs work closely with the nursing staff to ensure coordination of patient care. Provides physical support for patients/resident’s daily activities and personal hygiene, including bathing, dressing, grooming, transferring to/from bed, toileting, walking, standing, or exercising. Assists patients with daily medication and supplements. Ensures proper resident/patient diet based on their dietary restrictions. Examines patients for open wounds, bruises, or blood in urine. Notes and communicates patient/resident behavior and changes in mood. Cleans rooms and bed linens. Checks and records vital signs, including blood pressure, pulse, temperature, and respiration rate under the supervision of a licensed nurse. Answers to patient calls and needs. All other duties, as assigned. Certified Nursing Assistant (CNA) Qualifications and Skills: Good collaboration and teamwork skills with the nursing staff. Familiarity with symptoms and signs of improvement for a number of medical conditions. Ability to read, write, speak and understand English. State Certified Nursing Assistant License/Certification. Must be CPR Certified. Must be able to push, pull move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet.
