Director of Nursing (RN) - Skilled Nursing Home and Long Term Care We are looking for an RN to join our team as the Director of Nursing at King David Post Acute Nursing and Rehablitation (formerly Menorah Park). This beautiful building is licensed for 322 with a wonderful team of caregivers. This full-time position would be eligible for our generous benefits package. The Director of Nursing (RN) oversees all nursing operations within the nursing home, ensuring the delivery of high-quality resident care and compliance with federal, state, and local regulations. This role provides leadership to nursing staff, manages clinical programs, and collaborates with the Administrator to support organizational goals. Key Responsibilities Leadership & Management Provide oversight, direction, and support to all nursing staff including RNs, LPNs/LVNs, CNAs, and support personnel. Develop staffing schedules that ensure adequate coverage and appropriate skill mix. Lead recruitment, onboarding, training, and performance evaluations for nursing personnel. Promote a positive team culture focused on quality care, accountability, and resident-centered service. Clinical Oversight Ensure residents receive appropriate, individualized nursing care. Oversee care planning, including assessments, care plan development, and interdisciplinary coordination. Monitor clinical outcomes and implement quality improvement measures. Review and manage incident reports, resident changes in condition, and follow-up actions. Regulatory Compliance Maintain compliance with state and federal regulations (CMS, state health department, OBRA), infection control standards, and facility policies. Prepare for and participate in regulatory surveys; lead corrective action planning when necessary. Maintain accurate clinical documentation in accordance with facility and regulatory standards. Quality Assurance & Performance Improvement (QAPI) Drive QAPI initiatives to enhance resident outcomes and facility performance. Analyze clinical data trends and create action plans to address identified issues. Conduct ongoing audits of nursing practices and documentation. Communication & Collaboration Serve as the primary liaison between nursing staff, physicians, families, and other departments. Conduct regular nursing meetings to communicate updates, expectations, and training topics. Work collaboratively with the Administrator and leadership team on operational and strategic initiatives. Resident & Family Relations Build positive relationships with residents and families, addressing concerns promptly and professionally. Ensure residents’ rights, dignity, and psychosocial needs are upheld at all times. Essential Qualifications: · Must possess, at a minimum, an unencumbered active RN license for the State of Ohio. · Nursing Degree from an accredited college or university, and a graduate of an approved RN program. (preferably a B.S.N.) · Must have a working knowledge of Word, Excel and Point Click Care · At least 3 years experience in a supervisory or clinical management nursing role Familiarity with Social Model vs. Medical Model of care. · Excellent written and verbal communication skills · Highly organized, ability to prioritize and manage nursing department · Familiarity with Medicare a plus · Excellent assessment and clinical decision making to prioritize resident needs · Working knowledge of assisted living regulations and survey process · Working knowledge of chronic disease management and the clinical needs of the geriatric population · Ability to collaborate with clinical partner. QUALIFICATIONS: Graduated of an accredited school of nursing and licensed by the state of NJ as a RN. Current CPR or certified within 90 days of hire Other certifications may be required for other areas of the organization POSITION SUMMARY: The RN assumes responsibility for a specific unit or area within the organization to include assessment (under the supervision of the DON/ADON), medication distribution, treatment clinical documentation, and supervision of CNA’s and other staff. PART I: JOB RESPONSIBILITIES AND STANDARDS 1. Uses dual identifiers to confirm resident/guest identify prior to medication administration and treatments. 2. Practices SBAR (situation, background, assessment, recommendation), the preferred method of communication when interacting with members of healthcare community. 3. Encourages resident/guest to be involved in their own care when possible. 4. Receives full and detailed report from the outgoing nurses to ensure proper communication and necessary follow-up of resident’s/guest’s care. 5. Recognizes and responds competently to changes in resident’s condition and documents appropriately. 6. Initiates Plan of Care on admission. Reviews, evaluates & updates plan & interventions as needed. 7. Interacts and partners with all multidisciplinary service teams, internal and external, to assure continuum of care. 8. Practices Standard Precautions and adheres to infection prevention strategies. 9. Demonstrates compliance to Resident’s Rights. 10.
