Assists with repositioning and transferring residents. Ambulates and transports residents. Records and totals fluid intake and output. Weighs residents. Documents on flow sheets. Assists with admission, discharge and postmortem care. Responds to call lights in a timely manner. Performs perineal care and uncomplicated colostomy care. Reports all observations & assessments to the Team Leader/Nurse Manager/Charge Nurse. Takes and records vital signs. Makes beds and keeps resident’s units clean and in order. Prepares room for housekeeping after resident discharge. Skill / Experience Requirements . · Current (insert State) CNA Certification. · Previous Long Term Care experience a plus. New Pay scale Rotation every other weekend POSITION PURPOSE: Organizes and directs assigned unit duties to provide quality nursing care to residents. ESSENTIAL FUNCTIONS OF POSITION: • Responsible during the shift for the total nursing care of residents in the assigned unit. • Assumes responsibility for compliance with Federal, State, Local, and company regulations. • Explains procedures and treatments and care plan to resident. • Administers prescribed medications and treatments in accordance with approved nursing techniques. • Transcribes physician's orders. • Charts progress notes in an informative, factual manner that reflects the care administered as well as the resident's response to care. • Receives /gives report from/to nurse he /s he is relieving/coming on and records sufficient infonnation and implements appropriate follow-up action as necessary • Follows established procedures for charting and reporting all reports of incident/accidents for res idents and employees. • Conducts frequent rounds to evaluate effectiveness of care delivery. • Prepares equipment and a ids Physician during treatment and exami nation of resident. • Maintains awareness of comfort and safety needs ofr esident. • Observes resident, records significant conditions and reactions, and notifies supervisor or Physic ian of resident ' s condition and reaction to drugs, treatments, and significant incidents . • Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from nonnal and assess condition of resident. • Responds to emergency situations based upon nursing standards, policies, procedures, and protocol. • Assists in completion of the care plans (acute and long term) and is involved in the care conferen ce as deemed necessary. • Participates, reports to DON/ Administrator and conducts (as appropriate) investigations related to any alleged abuse. • Ensures that documentation i·s reflective of the care plan and that the approaches on the care plan are being followed. • Assures that assigned CNAs are aware of the resident care plan, and ensure that the nursing assistant refers to the care plan prior to administering care. • Monitors that CNA documentation is completed daily . • Takes report from CNAs to facilitate accurate documentation in resident medical record. • Follows company policy and procedure in the event a restraint must be utilized. • Coordinates residents' rehabilitation program with therapists to ensure 24 hour rehabilitation needs are met. • Identifies residents at risk for nutritional problems and implements preventative measures. • Records new/changed diet orders. Must have CNA license and KMA certification for consideration $18.50 an hour plus shift differential Work every other weekend POSITION PURPOSE: Ensures proper dosage and distribution of medication in accordance with State regulations, takes vital signs, responds to emergencies, records information in a resident's medical record, assists in management of the medical record as needed, and any other assigned duties to support the medical department as they have certification or documented training on. ESSENTIAL FUNCTIONS OF POSITION: • Maintains adequate stock of drugs, ensures that storage of drugs is in compliance with prescribed conditions of temperature and humidity, and verifies that all drugs are labeled properly. • Prepares medication for residents. • Distributes medication and instructs residents in proper application/consumption of medications per the directions of the physician. • Records each distribution of medication in resident's medical file. • Takes vital signs and records on the appropriate document. • Can be responsible for documenting, faxing, following up, and providing documentationrequired for all authorizations for each entity requiring prior authorization for any and all medical care. • Perform glucose testing by glucometer as directed. • Perform additional duties as assigned.
