DON Director of Nursing Services

Position Description: Under the direction of the Administrator, the Director of Nursing Services is responsible for the administration and management of Nursing Services to residents in accordance with orders of the physicians and total needs of the residents. Responsible for 24-hour supervision of Nursing Services. Qualifications: • A graduate from an accredited school of professional nursing • Current licensure as a Registered Nurse in the state in which practicing • Experienced or trained in nursing service administration, rehabilitation, psychiatric or geriatric nursing • One or more years of demonstrated ability in nursing administration and/or supervision in a health facility in the last 5 years • Supervisory or operations management experience, a strong understanding of skilled nursing standards of care, and federal and state regulations including Medicare, Medicaid, and Managed Care reimbursement rules • Demonstrates knowledge of and ability to apply basic principles of nursing care and teaching • Demonstrates leadership ability and capability to work effectively with all levels of personnel • Maintains an interest and knowledge of current trends and new developments in nursing and health administration • Demonstrates interest and ability to work effectively with the public • A desire to work with geriatric and disabled persons • Skills for critical-thinking and problem-solving • Ability to generate enthusiasm and cooperation from others • Ability to develop and maintain good interpersonal relationships • Possession of good emotional and physical health • A high level of professionalism and excellent communication skills • A genuine interest in and respect for people and a desire to be of service to them • Ability to accept and work with persons with different social and cultural traits • Ability to work in a long-term care environment General Duties and Responsibilities GENERAL • Assumes ultimate responsibility for coordinating plans for the total care of each resident which comply with physician’s orders, governmental regulations, and facility resident care policies • Develops, maintains, and upgrades nursing service objectives, standards for nursing practice, nursing procedure manuals and written job descriptions for each level of nursing personnel • Participates in the recruitment, selection, hiring, orientation, training, and dismissal of nursing personnel • Assists in controlling nursing service hours and supply costs, including recommendation for the number and type of nursing personnel to be employed and nursing service equipment and supply needs • Participates in admission and discharge planning for residents • Communicating with doctors, residents, and family members about resident health • Maintains contact with physicians as necessary according to resident needs, facility policies and governmental regulations • Implementing new policies and procedures and training nurses • Evaluates information to be shared with outside nursing personnel and allied services for continuity of resident care • Assess community resources that may be utilized to improve resident care • Maintains good interpersonal relationships with residents and their families, nursing personnel, allied professional services and the community • Periodically visits residents to ensure maximum care and to ascertain need to additional or modified services • Participates in professional organization activities, identifies, and meets educations need of staff through meeting and in-service classes • Attends and participates in all required meetings, conferences and policy making committees in the facility • Promotes a safe environment within the facility, which includes fire and accident prevention, maintenance of sanitary conditions and assurance that all nursing personnel understand their roles in the event of a disaster or other disruption of services • Assures that an adequate orientation and in-service training program is provided for all nursing personnel • Confers daily with the Administrator regarding current status of nursing services and submits written reports as requested SUPERVISION • Assigns duties and delegates responsibilities to nursing personnel according to their abilities • Evaluates performance of nursing personnel reporting to her/him and maintains evaluation records • Coordinates nursing service with other resident care services provided; such as physician, dietary, occupational, physical and speech therapies, resident activities, social services, medical records and business office • Assures that a resident Plan of Care is established for each resident and that the plan is reviewed and modified as needed • Encourages staff development and professional growth • Assures adequate 24-hour nursing coverage in the facility each day • Designates a responsible professional nurse to be in charge of the nursing service during the absence of the Nursing Director • Provide close supervision and direction to the Charge Nurses to continually improve the nursing care of residents • Participate in the Quality Assurance Committee Meeting • Reviews and corrects findings and deficiencies of Pharmacy Consultant, Department of Health Services, Dietary Consultant, Medical Records Consultant, Quality Assurance Team and prevent reoccurrence by implementation of recommendations and follow through • Maintains quality assurance in facility by frequent nursing rounds and continuous assessment of resident conditions from admission through discharge • Assumes responsibility for investigation of alleged abuse or unusual occurrence as outlined within our established policies and procedures CONSUMER SERVICE • Presents professional image to consumers through dress, behavior, and speech • Adheres to Company standards for resolving consumer concerns • Ensures that all resident rights are protected

CNA (Certified Nursing Assistant)

Sign-on Bonus: $1,000 Open Interviews on Wednesdays 11am - 2pm and Thursdays 1pm - 4pm A nursing assistant is responsible for providing routine nursing care in accordance with established policies and procedures and as may be directed by the Charge Nurse, RN Supervisor, Director of Nurses or Administrator, to assure that the highest degree of quality resident care can be maintained at all times. QUALIFICATIONS • Currently enrolled in a school for nursing assistants or have a license as a certified nursing assistant. • A minimum of 6 months experience as a certified nursing assistant or clinical experience while enrolled in a nursing assistant certification class. • Must possess a sincere desire to serve the ill, aged and infirm. • Ability to read, write and speak the English language. • Possess the ability to deal tactfully with personnel, residents, family members, visitors, and the general public. • Willingness to work harmoniously with other personnel. • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level they are currently functioning. • Must be in good general health and demonstrate emotional stability. • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility. GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Perform all duties as assigned and in accordance with facility’s established policies and procedures, nursing care procedures and safety rules and regulations. • Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care. • Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30° for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. RN Supervisor (Registered Nurse)

Sign-On Bonus: $2,000 A licensed professional nurse directly under the supervision of the Director of Nursing Services. The RN Supervisor is to assist the planning, organizing, developing and directing of nursing service to assure that the highest degree of quality resident care can be maintained at all times. QUALIFICATIONS • Graduate of an accredited school of nursing. • Current nursing licensure in the state in which practicing. • At least one year of experience in supervision. • Strong qualities of leadership, communication and organizational skills. • Ability to communicate effectively in English through both speech and writing. • Possess mental, emotional and physical health. GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Assure that residents have a clean, safe, orderly and comfortable environment. • Initiate resident Plans of Care on admission. • Assist in implementing recommendations from consultants and the Quality Assurance team as directed by the Director of Nursing Services. • Assist in developing, reviewing, revising an updating resident Plans of Care for individual residents. • Ensure that notification is given to the resident’s attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., or has a significant change in condition as outlined within our established policies. • Review complaints and grievances form families and notify the Director of Nursing Services as necessary. • Evaluate emergencies and assist in implementing emergency measures. • Assure that the unit area is maintained in a clean and safe manner for resident comfort and convenience, by ensuring that necessary equipment and supplies are maintained to perform nursing care. • Assist in notification of physician of any resident incidents/unusual occurrences. Treatment Nurse LVN ( Licensed Vocational Nurse )

Under the direction and supervision of the Director of Nursing Services, the Treatment Nurse is responsible for all the treatments that are prescribed by the attending physician for all residents in the facility. QUALIFICATIONS • Graduate of an accredited school of professional nursing. • Current licensure as a Registered Nurse or a Licensed Vocational Nurse in the state in which practicing. • Possess one or more year’s experience in a skilled nursing care facility. • Ability to read, write and speak the English language. • Possess mental and physical health. GENERAL DUTIES AND RESPONSIBILITIES: CLINICAL • Assess resident’s skin condition by doing complete body assessment on admission, re-admission and return from day, night or extended leave of absence. • Initiate the Nurse’s Skin and Wound Progress report upon identification of any skin problem and/or pressure ulcer. • Notify attending physician promptly when any skin condition is identified. • Initiate all treatments immediately as prescribed by attending physician. • Ensure that all skin conditions have corresponding treatment orders. • Ensure that all sites of any skin problems are identified and documented on the nursing assessment form upon admission. • Photograph all newly admitted residents with any skin problems and with stage 2, 3, and 4 pressure ulcers. Indicate date photograph was taken. • Photograph all residents with any skin problems and with stage 2, 3, and 4 pressure ulcers upon discharge, except in an emergency transfer. • Ensure that all skin problems and pressure ulcers are documented on the resident’s Plan of Care. • Perform a weekly skin assessment on all residents and submit findings to the Administrator, Director of Nurses, MDS Coordinator, Dietary Consultant and Physical Therapist. • Document skin assessment findings during weekly assessment on weekly Nurse’s Skin Wound Progress Report form, inform physician as needed and enter on resident’s Plan of Care. • Notify attending physician and Director of Nursing Services promptly of any deterioration of any skin problem and non-response to treatment. • Notify Dietary Consultant promptly on any need for dietary intervention. • Notify Physical Therapist promptly on any need for wound management. • Transcribe and document treatment orders on treatment sheet. • Audit and check treatment sheet recaps on a monthly basis. • Audit missing documentation on treatment sheet at the end of each month. • Order medication and treatment from pharmacy as prescribed by the attending physician. • Consult with skin care consultant regarding need to initiate or discontinue special bed. • Responsible for daily charges of equipment and supplies. • Review monthly charges before submission to Business Office. • Monitor oxygen supply. • Check and request supplies weekly from central supply personnel. • Change drainage bags weekly and PRN, or as per facility policy and procedure. • Responsible for taking cultures as ordered by the attending physician. • Attend in-service classes, meetings and seminars as required. • Make monthly rounds with skin care consultant. • Keep treatment carts clean at all times. • Notify Director of Nursing Services promptly of any skin problem that developed in the facility. • Others as may become necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator.
