Throughout this project, you will gain hands-on experience in healthcare data transformation, learning how raw claims and financial data flow through a modern pipeline into auditable payment platforms. By participating in the full project lifecycle—from initial discovery to go-live—you will develop practical skills in requirements gathering, technical documentation, and cross-functional collaboration. Practice Transformation Specialist, Flex

$60,000 - $85,000 / year
Practice Transformation Specialists serve as Aledade's frontline champions, building meaningful relationships with healthcare providers while gaining invaluable insights into our partner practices' evolving needs and success stories. As part of our innovative Flex Field Practice Transformation team, you'll deliver scalable, expert-level support that ensures every practice receives the consistent, high-quality guidance they need to thrive in value-based care. This dynamic role puts you at the heart of healthcare transformation, where your direct impact helps practices achieve better patient outcomes while driving meaningful change across the healthcare landscape. The position requires domestic travel within the United States up to 25% across the year. Outside of these travel periods, the role is entirely remote. Candidates should be comfortable working remotely or from home anywhere within the US, ensuring flexibility and adaptability in both settings to successfully fulfill the responsibilities of the role. \n Primary Duties: Direct Practice Support: 80% - Responsible for supporting markets with staffing capacity for new and existing practices. This includes onboarding practices to Aledade, coaching on our technologies, and leading initiatives to ensure practices meet the goals outlined in their jointly developed Practice Transformation Plans. ACO Training and Development: 10% - Work to assist in training office staff and physicians & providers (in coordination with Aledade's Value Based Care Curriculum) on processes and technologies. Provide continual assessment of practice’s and staff’s training needs. Develop and nurture lasting relationships and engagement with physicians, providers, and healthcare system executives to support and enhance financial and quality performance across performance metrics. Customer Success: 10% - Listening to practices concerns, relaying important information to Manager/Leads to understand opportunities for improvement. Minimum Qualifications: 4-6 years of significant and relevant work experience in medical practice management in lieu of educational requirements may be accepted, particularly with significant administrative experience in a clinic setting. 3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes. Preferred KSA's: Excellent oral and written communication skills (i.e., public presentation skills), organizational and project management skills, and team-building skills. Familiarity with process mapping and work-flow analysis tools. Excellent computer skills and willingness to learn additional software applications. Physical Requirements: Sitting for prolonged periods of time. Extensive use of computers and keyboard. Occasional walking and lifting may be required. This position encompasses comprehensive quality assurance responsibilities, including conducting thorough reviews of practice submissions to CMS to ensure complete alignment between submitted documentation and application details, accurate routing of submissions to the appropriate Accountable Care Organization (ACO), and full correspondence between executed Participation Agreements (PAs) and corresponding CMS submissions. Additionally, the intern will provide vital support in distributing reshuffle Participation Agreements to existing network practices - a high-volume, time-sensitive process requiring additional team capacity during peak periods. Summer Internship - Policy

The Policy summer intern will help the team track legislation at the state level related to primary care investment and other issues Aledade cares about (value-based care, etc.). They will help us plan our annual fall Hill Day where we invite Aledade physicians to come to DC to meet with their legislators to advocate for primary care and accountable care. The intern will also help with small projects throughout the summer, such as preparing decks/presentations, researching different issues in policy, possibly writing short articles. The Population Health Manager is a collaborative, strategic clinical leader who helps drive the success of Aledade’s value-based care initiatives by championing health equity and leveraging data-driven insights. This role specifically focuses on high-impact, low-performing outlier practices that have a unique opportunity to improve the population health of their communities by addressing health equity. This role facilitates high-level change management, performance improvement, and the implementation of specialized programs, such as those related to Social Determinants of Health (SDOH). The Manager is an innovative self-starter responsible for developing new population health initiatives and forging external partnerships to secure resources for patients. This role must gain credibility with multiple stakeholders in order to facilitate the implementation of strategic plans that optimize patient outcomes. This position requires 50% travel to partner practices to conduct in-depth assessments and facilitate change. \n Primary Duties: Change Management & Strategic Implementation: Develop and execute strategic change management plans for new, complex population health initiatives. Lead discussions with practice leadership to identify performance barriers and co-create innovative, practice-specific solutions. Manage and own the "Gemba Walk" process, conducting comprehensive on-site assessments and developing strategic action plans to drive practice transformation. Serve as the primary subject matter expert for the implementation of new population health programs and initiatives across practices. Performance Improvement & Data Utilization: Lead the analysis of complex practice performance data to identify trends and opportunities for improvement in quality metrics, patient engagement, and cost efficiency. Research and analyze public health data and community-specific challenges that impact a practice's ability to achieve value-based care (VBC) outcomes. Translate high-level data and analytics into actionable, strategic insights for practice leadership and staff. Drive the implementation and management of specialized Social Determinants of Health (SDOH) solutions at the point of care to address health equity.
