Essential Job Function Applies knowledge of healthcare systems, resources, and patient needs to guide patients through their medical journey, ensuring access to necessary services and support. Expedites, prioritizes, synchronizes and mediates (in real time) clinical issues, removes barriers to discharge and addresses delays in care; identifies potential issues so staff can prioritize patient-care activities and discharges and mitigate potential bottlenecks before they occur. Confers with multi-disciplinary teams to identify effective strategies/approaches to reduce patient length of stay, handle complex discharges, conditional discharge orders and related issues ensuring patients have a safe discharge.
