In this role, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. Your primary objective will be to assess the overall health and well-being of member beneficiaries to ensure accurate and comprehensive risk adjustment coding, leading to greater value-based care. This role is 1099 (PRN) and offers tremendous flexibility and opportunity for those who are balancing competing priorities. NP Responsibilities: In-home Clinical Assessments Travel to members’ homes and conduct comprehensive health evaluations Conduct Annual Wellness Visits and health assessments for members with chronic conditions. Perform: Medical history and physical examinations Review of systems Vital signs collection Medication review and reconciliation Possible preventive health screenings Hemoglobin A1C Spirometry KED (Kidney Health Evaluation) DRE (Diabetic Retinal Exam) FIT Patient Education & Engagement Deliver preventive health education and counseling opportunities. Educate patients on ongoing health monitoring, medication adherence, and chronic condition management. Support informed decision-making and ensure true, informed consent. Engage in goals of care discussions when appropriate. Care Coordination & Quality Support Identify clinically relevant diagnoses for care management and risk adjustment documentation. Communicate findings used to generate post visit summaries for the primary care provider. Identify and help close quality care gaps (e.g., screenings, labs, follow-ups). Collaborate enthusiastically with the broader care team to enhance clinical quality and patient experience. Compliance & Professional Standards Adhere to HIPAA regulations and safeguard protected health information (PHI). Maintain accurate, timely, and compliant EMR documentation. Perform additional duties as assigned in alignment with organizational goals. An enthusiastic collaborator contributing to the enhancement of care delivery Providers are expected to commit a minimum of 30 hours per month Locations: Jackson County, OR Travel Requirements Daily travel expectations remain within a 55-mile radius of the assigned home location. Job Title: Family Medicine Physician Assistant/Nurse Practitioner Job Summary: We are seeking a qualified Physician Assistant or Nurse Practitioner to provide comprehensive primary care services in a family medicine setting. This role involves the evaluation, diagnosis, and management of patients across the lifespan, with a focus on preventive care, chronic disease management, and patient education. The provider will work collaboratively with physicians and interdisciplinary team members to deliver high-quality, patient-centered care. Key Responsibilities: Provide comprehensive primary care to patients of all ages, including assessment, diagnosis, and treatment of acute and chronic conditions. Conduct routine physical exams, wellness visits, and preventive health screenings. Develop and implement individualized treatment plans and manage medications. Order, interpret, and review laboratory tests, imaging studies, and other diagnostic procedures. Educate patients and families on health promotion, disease prevention, and treatment plans. Coordinate care with physicians, specialists, and ancillary services as needed. Document patient encounters accurately and in a timely manner in compliance with regulatory and organizational standards. Participate in quality improvement initiatives and continuing professional education. Qualifications: Graduate of an accredited Nurse Practitioner or Physician Assistant program. Current national certification as a Nurse Practitioner (FNP preferred) or Physician Assistant. Active state licensure and prescriptive authority. Strong clinical assessment, diagnostic, and decision-making skills. Excellent communication, collaboration, and interpersonal abilities. Commitment to patient-centered, evidence-based care. Job Title: Per Diem Inpatient Mental Health Physician Assistant/Nurse Practitioner Position Summary: We are seeking a qualified Per Diem Inpatient Mental Health Advanced Practice Professional (PA or NP) to provide psychiatric care to hospitalized patients. This role supports inpatient behavioral health services by delivering assessments, treatment planning, and ongoing management for patients with acute and chronic mental health conditions in a hospital setting. Key Responsibilities: Perform psychiatric evaluations, mental status examinations, and risk assessments for inpatient patients. Diagnose and manage mental health conditions, including mood disorders, psychotic disorders, anxiety disorders, substance use disorders, and behavioral disturbances. Develop and implement individualized treatment plans, including medication management and therapeutic interventions. Collaborate with psychiatrists, hospitalists, nursing staff, social workers, and case management teams to coordinate care and discharge planning. Monitor patient progress, adjust treatment plans, and respond to changes in clinical status. Provide patient and family education related to diagnoses, medications, and treatment plans. Document patient encounters accurately and in compliance with medical, legal, and regulatory standards. Participate in interdisciplinary rounds and quality improvement initiatives as assigned. Qualifications: Master’s degree in Nursing (Psychiatric-Mental Health NP preferred) or completion of an accredited Physician Assistant program. Active NP or PA licensure in the state of employment. National certification as an NP or PA; psychiatric certification required or preferred based on role. Prior inpatient mental health or psychiatric experience preferred. Strong assessment skills, clinical judgment, and ability to manage acutely ill patients. Excellent communication, collaboration, and documentation skills. Work Environment: Inpatient psychiatric unit or medical hospital setting with behavioral health services. Per diem, as-needed scheduling, including potential evening, night, weekend, or holiday shifts. Fast-paced, interdisciplinary team environment focused on patient safety and quality care.
