Implement and improve work methods and procedures to ensure policy-aligned admissions. Foster and maintain positive relationships with referral sources. Participate in quality assessment and performance improvement activities. Maintain comprehensive knowledge of community resources and assist referral sources when needed. Perform additional duties as assigned by management. Qualifications: Current LVN license with a minimum of one (1) year of experience in a healthcare office, preferably in hospice. Proficient in computer systems and documentation. Strong customer service and time management skills. Working knowledge of healthcare regulations, CHAP standards, and payer practices.
