This is a statewide position and will report to their nearest DHW location. This position may be eligible for limited telework after successful completion of probation and necessary training. Observe care delivery and conduct interviews to ensure services meet state and federal standards. Explain available services, and complete comprehensive evaluations to help individuals access Home and Community-Based Services (HCBS). Serve as a trusted resource for participants, families, and providers. Answer questions about HCBS and supplemental services, assist with provider-related concerns, clarifying service authorizations and eligibility. Support the team and maintain program integrity through documentation and correspondence. Participation in staff meetings and general office duties that keep operations running smoothly. Conducting regulatory compliance reviews through client sampling. Assess clients' health , emotional, and rehabilitative needs. Applying critical thinking and clinical judgement to evaluate care quality, writing clear, concise, and legally sound reports. Description The Licensed Practical Nurse (LPN) is responsible for identifying and reporting patient needs through data collection, planning, intervention and evaluation and delivery of appropriate care according to nursing standards and professional practice as outlined by the Nurse Practice Act for LPN’s. Collection of data is via interview, observation, auscultation and palpation. Through documentation and oral communication, essential information is shared with healthcare team members and other customers common to the department. Requirements Hold a current LPN license in the State of Idaho. Ability and willingness to demonstrate and maintain competency as required for job title and the unit/area(s) of assignment. Excellent communication skills to include written and verbal comprehension and expression. Ability and willingness to work with patients’ growth and development needs particularly related to the age of patients in the unit/area(s)of assignment. Ability to manage a chaotic work environment related to changing patient needs, including working with patients with acute, chronic, and complex disease processes, and those that are dying.
