We're looking for a compassionate Certified Nursing Assistant (CNA) in Indian Springs, NV, to provide assistance in daily living and personal care services in patients’ homes in accordance with a physician established care plan. This role is a 1099 contract opportunity, with care being provided at the patient’s residence. We are generally flexible with scheduling, but ultimately current scheduling needs for this position will be established between the CNA, patient, and case manager. We are currently looking for the following needs: Monday - Sunday 7am - 7pm OR 7pm - 7am Please be aware that offers are contingent upon passing a background check. #INDPLIC \n Responsibilities Work with the patient in their home to provide one-on-one service in a comfortable and familiar setting. Assist with the patients’ activities of daily living as well as personal care. Provide companionship care and basic emotional or psychological support. Meal preparation Light housekeeping Transport and accompany patients to doctor or hospital visits. Adhere to the patient’s established plan of care. Execute the physician prescribed plan of care and maintain compliant documentation of care provided in the system of record. Monitor vitals (vital signs, temperature, respiration, etc) according to plan of care. Remind patient of and administer medication as prescribed. Maintain records and documentation of daily services. Communication with the patients’ family, doctors, and other prescribed individuals regarding the health and care of the patient. Minimum Qualifications Current CPR certification Current First Aid certification TB test & physical examination Ability to monitor vital signs. Good interpersonal skills Caring and compassionate personality Hand-eye coordination and manual dexterity Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line. Ideal teammates will have experience in dialysis, get energized by a start-up type environment and improving the lives of patients, and have a track record of finding ways to Get Stuff Done (GSD). Teammates must be comfortable working autonomously and have demonstrated the ability to collaborate with cross-functional resources across the organization. What you can expect: In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. DSD RNs will troubleshoot, and make sound decisions for our patients related to their dialysis treatment and will occasionally partner alongside the SNF staff to coordinate care. Primary work schedule will be 3-4 days a week with some travel to other SNF clinics within your region (depending on business and patient needs). Alternating Saturdays will also be required. As a DSD Services RN, you will be provided the opportunity to conduct impactful and meaningful education to patients and their families in the SNF setting. The role includes supervising Dialysis Technician who are tasked with delivering tandem care to meet the capacity and efficiency goals of the program. Autonomy and Critical Thinking.
