Summary: Provides personal care, light housekeeping and assistance with ADLs (Activities of Daily Living) as outlined in each SE participant’s plan of care in participant homes as well as at the PACE center; exercises independent judgement; reports any changes in participant status to the IDT; participates in carrying out infection control precautions and increased cleaning and disinfecting of the PACE center as assigned and in accordance with current CDC guidelines Responsibilities Under the supervision of the RN, assists with the Activities of Daily Living (ADL) needs of participants both at the PACE center and in participants’ homes (i.e., community, Assisted Living Facilities, Rest Homes, Supportive Housing programs, etc.) Contributes to the development of a care plan for participants through interaction with other members of the Interdisciplinary Team. Collaborates with members of the Interdisciplinary Team to assure appropriateness and continuity of care. Carries out non-skilled treatments including, but not limited to vital signs, transfers, toileting, bathing, dressing at the PACE Center and in the community. Assists the Supervisor, Recreational Activities and other activities staff with individual and group programs by helping to plan individual treatment programs, increasing participants’ motivation to participate, assisting participants to participate when needed, and assisting with evaluation of program effectiveness. Assists registered therapists and certified therapy assistants with treatments and participant-specific activities which are ordered for each participant and assigned by the registered therapist including, but not limited to, positioning, transfers, ambulation, and exercises. Maintains a clean and safe working and/or living environment in the PACE center and/or participants’ homes. Assists with meal and snack preparation, serving, feeding as needed, and clean-up. Uses safe techniques in all interactions with participants at the PACE Center and in participants’ homes. Provides accurate and timely documentation in the EMR and other systems as required by SE policies and procedures and/or as assigned by supervisor. Participates in carrying out schedule of daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines for increased precautions Actively participates in distribution of work for health aides to ensure care needs of participants and site tasks are completed timely and appropriately including but not limited to community-based care and center-based tasks, as assigned Actively participates in regular team meetings for health aides at respective site Participates in carrying out infection control precautions and increased cleaning and disinfecting of the PACE center as assigned and in accordance with current CDC guidelines Performs all duties in accordance with FH and Summit ElderCare policies and procedures. Qualifications Education: Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but we will accept equivalent work experience in lieu of certificate. Certification: Certification as a Home Health Aide or Nursing Assistant is desired. Reliable Transportation Required. Experience: One year of experience working with a frail and/or older adult population preferably in a PACE program. If under one year, must complete Summit Eldercare training program for health aides prior to working with participants independently. Provides quality care to children and young adults in accordance of the Nurse Practice Act of Massachusetts. Responsibilities •Assess and coordinate healthcare services for Program Manager. Institute appropriate follow up and communicate all medical issues to the program Residential Registered Nurse and any other appropriate Seven Hills Pediatric Center at Hopedale staff members.•Assist individuals in designated programs to their medical appointments and with transportation as recommended by the Program Manager, Registered Nurse, or the Nurse Practitioner.•Document all nursing interventions and changes affecting the children in appropriate areas.•Review the plans of care for current knowledge of programming as required by the Department of Children and Families and/or other agencies.•Maintain current medication and treatment orders noting any side effects and/or adverse effects and report to appropriate individuals: MD, NP, Program Manager or program Residential RN, and other designated Seven Hills Pediatric Center at Hopedale staff. Assist in training staff in medication policies and procedures.•Maintain health records according to various government agencies as required. Ensure that information is communicated to residential staff and school program regarding recommendations and treatment.•Consult with residential Registered Nurse for supervision and direction as needed to ensure appropriate medical health care is provided for participants.•Attend staff meetings to present all current medical/health concerns and any upcoming medical appointments to team.•Complete all mandatory in-service education in a timely manner and attend any educational programs at the recommendation of the Program Manager.•Implement procedures and services including medical interventions, or additional duties as assigned by the Program Manager or residential Registered Nurse.•Ensure that residents are sufficiently observed to maintain their safety and optimal health. Vacation Cash-Out Option 3 Paid Personal Days 11 Paid Holidays Accrued Paid Sick Time Responsibilities Provide medical consultation to MCCN team members regarding acute changes in Enrollees’ status Serve as liaison to and consult with the MCCN Medical Director Conduct medication reconciliations during comprehensive assessments and after inpatient, ED, or 24-hour setting discharges Coordinate with PCPs to verify discharge medications, address discrepancies, and promote adherence Educate Enrollees on proper medication administration and identify support systems to aid compliance Support Care Coordinators in ensuring access to appropriate medical and specialty care Review and complete medical sections of Enrollee assessments and collaborate on LTSS components Participate in interdisciplinary team meetings to promote effective communication Assist with staff orientation and training; provide coaching on medical conditions, risk factors, and CP team priorities Follow all MCCN policies, protocols, and evidence-based practices while assisting with staff training Apply a person-centered approach in planning and delivering care in coordination with Enrollees and their support systems Address medical risks and emergencies with appropriate interventions; develop medical protocols as needed Complete all documentation and reporting in a timely and compliant manner Maintain strong communication with staff, Enrollees, and care team members to foster collaboration Ensure adherence to HIPAA, agency policies, and performance standards Attend required trainings, staff meetings, and supervision; maintain all certifications and licenses Treat all clients with dignity and respect in alignment with MCCN’s Human Rights policy Perform all duties in accordance with agency standards and expectations Qualifications Education & Experience: Registered Nurse licensed in the Commonwealth of Massachusetts Minimum of 4 years of experience working with adults in a community-based mental health, substance use and/or physical health setting Demonstrated knowledge base and judgment necessary for independent clinical decision making. Skills and Knowledge: Excellent verbal and written communications skills. Strong interpersonal and customer relations skills required. Ability to use Electronic Health Records (EHR) Systems to document and coordinate services. Excellent organizational, time management and problem-solving skills. Strong computer knowledge, including proficiency in contemporary Windows operating systems and Windows office suites with an emphasis on Word and Excel; ability to learn new systems; experience entering and working with data; and comfort and experience using mobile technologies. Knowledge regarding psychiatric rehab and understanding of recovery model. Strongly prefer that a candidate will have a demonstrated understanding of and competence of Health Equity and in serving culturally diverse populations. Must be able to perform each essential duty satisfactorily. Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport members.
