Description: The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Job Functions:

Provides direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end of life care. Implements current nursing practice following a comprehensive assessment and the plan of care. Provide accurate and timely documentation of patient services to reflect the plan of care. Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care. Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include Hospice Aide, OT, PT, SW, SLP, Dietician, or Clinical Supervisor. Uses and prepares equipment and supplies effectively, adhering to aseptic technique. Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications. Participates in personal and professional growth and development. Also participates in organization’s quality assessment performance improvement teams and activities. Performs other duties as assigned by the registered nurse.

Qualifications:

Graduate of an accredited practical nurse or vocational nursing program. Three years’ nursing experience. Community health/hospice or medical/surgical experience is preferred. Currently licensed as an LPN/LVN in the state of practice. Complies with accepted professional standards and practice. Demonstrates good verbal and written communication, and organization skills. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Understands philosophy of hospice concept. Also understands the needs of the terminally ill.

You’ll get to advocate for the patients and work one-on-one to provide them with the care that they need. This role will collaborate with CNA’s, HHA’s, Case Managers, and the patient’s physicians. Offers are contingent upon passing a background check. #INDPLIC \n Responsibilities: Execute physician prescribed plans of care. Complete compliant documentation of the care provided in the system of record. Manage and administer prescribed medication, treatment, and therapies. Conduct patient assessments, coordination of care. Perform various activities associated with daily living. Monitor vitals and GI intake and output. Assess skin integrity and administer wound care. Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression. Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients. Provide education and training to the patient, their family, and/or the Home Health Aide. Work with patients to be done one-on-one in the patients home to provide for a comfortable environment. Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants. Perform other personal care services as necessary to meet the patient’s needs. Minimum Qualifications: Education, licensing, and certification: graduate of an accredited college or university, Associate or Baccalaureate School of Nursing Current State or Compact License as a Registered Nurse or Licensed Practical Nurse. Current CPR certification TB test and physical examination. Maintain required insurance. Good interpersonal skills. Hand-eye coordination and manual dexterity.
