Job Responsibilities

Working in post-acute facilities in the area as an LPN
Being accountable to follow and adhere to resident care plans
Tracking the wellbeing of residents, both physical and mental, and monitoring their vital signs

Answer residents’ call signals. Respond to door alarms, fire alarms, etc. Assist residents, alone or with assistance, with turning, repositioning, moving in and out of bed, toileting and personal hygiene (including monitoring of bowel and bladder incontinence), walking and exercise. Observe and report resident behavior and condition changes (including food/liquid intake/output, vital signs, skin concerns, weight and signs of injury or neglect), and communicate information to Health and Wellness Director.
