Job Responsibilities

  • Providing post-acute LPN nursing care to residents with varying medical needs
  • Tracking the vital signs, physical condition, and mental acuity of residents
  • Completing intakes of residents, including the documentation of medical histories and health concerns
  • Assisting with medications, including proper administration, verification, and monitoring
  • Keeping track of the medical records and charts of residents
  • Helping to maintain a safe, comfortable, and pleasant environment for residents
  • Ensuring proper storage and inventory of medical supplies
  • Setting up and assisting with immunizations, IVs, and phlebotomy procedures
  • Being accountable to follow and adhere to care plans for residents and all applicable policies and regulations
  • Supporting the training and orientation of new clinicians

Qualifications and Skills

  • Current Connecticut state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • Graduation from high school (or GED/equivalent)
  • At least 6 months experience working as a LPN (or similar role)
  • Ability to lift at least 50 lbs
