Job Responsibilities

Implementing best practice nursing care in post-acute residents in nearby areas
Monitoring the conditions of residents and providing updates to the nursing team
Measuring and documenting the basic vital signs of residents
Helping residents with grooming, feeding, toileting, and other basic activities of daily living (ADLs)
Promoting a clean and safe environment by ensuring best practices in hygiene and by clearing areas of hazards
Transporting residents around the facility and assisting them with other movements, as needed during the day
Interacting with residents on a daily basis to keep them engaged, provide comfort, and build trusting relationships
Being accountable to follow and adhere to care plans for residents and all applicable rules, regulations, and policies
Bridging communications between, and among, staff and residents (and their family members)

Qualifications and Skills

Current New Jersey Certified Nursing Assistant (CNA) certification
Demonstrated knowledge of nursing care procedures and clinical terminology
Detail-oriented and able to effectively communicate with medical professionals, residents, and family members
Ability to lift a minimum of 50 lbs

Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis. Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics. Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned. Routinely turn and position residents, as necessary. Observe residents carefully and report changes in condition to Charge Nurse. Participate in collecting data needed for the accurate completion of the MDS and the plan of care.
