Job Summary and Responsibilities

As our Wound Care Coordinator Nurse, you will specialize in comprehensive wound and ostomy care. You will elevate care quality, optimize patient outcomes, and ensure cost-effective management.

Every day you will provide expert consultation and direct care for complex wounds. You'll assess, plan, and evaluate individualized care, educate patients/staff on best practices, monitor outcomes, and ensure protocol adherence.

To be successful in this role, you will demonstrate strong critical thinking, communication, and a commitment to evidence-based practice.

  • Delivers and coordinates care to patients with alterations in skin/tissue integrity within the scope of practice and under the direction of physician or other clinician.
  • Performs patient intake, initiating clinical pathway on admission.
  • Collects patient history, reviews chart, lab reports, and other diagnostic/treatment data and develops nursing assessment.
  • Reviews, assesses, measures, and evaluates wounds and wound progress/status, assigns grade/staging of wounds, and may photograph wounds.
  • Contacts clinician to get orders.
  • Cleans and irrigates wounds, determines appropriate dressing, and applies or changes dressing as required; may assist with debridement and other wound treatments as requested.

  • Associates Of Science Nursing and two years of related work experience working with an interdisciplinary clinical team in one or more of the following clinical areas: Med/Surg, Gerontology, Home Health, or Wound Care that demonstrates attainment of the requisite job knowledge skills/abilities., upon hire and
