The Home Health RN plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their and communities.

ESSENTIAL RESPONSIBILITIES:

Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
Regularly re-revaluates patient nursing needs.
Initiates the plan of care and makes necessary revisions as patient status and needs change.
Uses health assessment data to determine nursing diagnosis.
Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and plan of care.

EDUCATION AND WORK EXPERIENCE:

Required Degree: Associate’s Degree in Nursing
Preferred Degree: Bachelor’s Degree in Nursing
Certificate(s): RN License and CPR certification, if required by state
Experience: 1+ Years

KNOWLEDGE, SKILLS, AND ABILITY:

Knowledge of home health practices, policies and procedures.
Ability to communicate effectively with patients and healthcare professionals.
Ability to perform thorough and accurate physical assessments to identify patient needs and health issues.
Knowledge of privacy regulations and maintaining patient confidentiality.
Ability to analyze complex patient data and clinical information to make informed decisions.

PHYSICAL REQUIREMENTS:

Ability to stand, sit, talk, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 100 lbs.
Ability to operate various equipment base on department.
The ability to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medical codes or alarms. Four hearing requirements listed: In person speech, Telephone, Other remote speech and other sounds.

Clarity of Vision:

For near visual acuity Clarity of vision at approximately 20 inches or less (i.e., working with small objects or reading small print), including use of computers.
For far visual acuity, Clarity of vision at 20 feet or more. This is not just the ability to see a person or object, but the ability to recognize features as well.
For peripheral vision, observing an area that can be seen up and down or to right or left while eyes are fixed on a given point.

Compensation Pay Range:

$38.74 - $49.26 per hour

Reasonable Pay Estimate

A reasonable estimate of the pay range for this position is $38.74 - $49.26 per hour. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
