Overview
Responsibilities
Patient Assessment & Admission
Assess and admit eligible patients for home health care services.
Conduct comprehensive OASIS assessments in accordance with regulatory and organizational standards.
Evaluate patient needs and determine appropriate levels of home health services.
Care Planning & Implementation
Develop individualized plans of care based on patient assessments, physician orders, and clinical standards.
Implement, monitor, and revise care plans to ensure high‑quality, safe, and effective patient outcomes.
Skilled Nursing Care
Provide skilled nursing services and comprehensive clinical assessments in the home setting.
Perform and manage physician‑ordered treatments including:
PleurX drain care
Finger-stick coagulation checks
Wound care and wound VAC management
IV therapy
Phlebotomy
Tracheostomy care
PEG tube care
Ostomy care
Telehealth monitoring and support
Teaching, Supervision & Counseling
Educate clients and caregivers on disease management, medication administration, treatments, and safety.
Provide supervision and clinical guidance to patients and family members to promote independence and self‑care.
Offer counseling to support patient understanding and participation in the care plan.
Interdisciplinary Collaboration
Work collaboratively with members of the multidisciplinary home health team to ensure coordinated, patient‑centered care.
Communicate effectively with physicians, therapists, social workers, and supervisors regarding patient status and care needs.
Clinical Documentation
Complete accurate, timely OASIS documentation in compliance with regulatory and agency standards.
Maintain thorough and clear clinical records to support quality care and continuity of services.
Professional Skills & Problem Solving
Utilize strong critical thinking and clinical judgment to identify patient needs and respond effectively.
Address patient and caregiver concerns promptly, employing sound problem‑solving strategies.
Additional Responsibilities
Participate in required weekend and holiday rotations.
Perform other duties as assigned to support optimal patient care and agency operations.
Qualifications
- Minimum of one (1) years recent experience as a professional nurse in Home Health.
- Valid DL With No More Than 3 Moving Violations
Credentials
- RN - Registered Nurse
- BLS - Basic Life Saving certification (American Heart Association)
Education
- Graduate of an accredited school of nursing
