Responsibilities: The Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting. Qualifications: Education

  • RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Upon Hire Required
  • CPR - Cardiac Pulmonary Resuscitation Upon Hire Required
  • DL NUMBER - Driver License, Valid and in State Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation Upon Hire Required

Responsibilities Include: Guides and directs and supervises the activities of the certified nursing assistants and LPN'. Shares responsibilities for conducting admission assessments, MDS and care plans with the Nurse Manager and the other staff nurses. Reviews, modify as needed, patients’ care plans in collaboration with patient, family/designated representative and the interdisciplinary team. Attends care planning and discharge planning review meetings. Provides patient/family education during stay and to prepare for discharge. Ensure that the rooms/beds assigned to new admissions, In-house transfer are ready to accept the patient. Conducts daily rounds with the Nurse Managers.
