About Thrive: Thrive of Lisle is a newer 74 bed transitional care facility. Our sole focus is on the short-term patient population, and we do not offer memory care or long-term care. We have a large in-house therapy team and most of our patients go home within 14 days (or sooner). We cater to the needs of this motivated generation with larger beds, nicer linens, fine dining cuisine (we even have a brick oven for pizzas) ...all in a tastefully designed modern decor. Here, you will perform an integrated role incorporating clinical care facilitation, utilization management and discharge planning, in order to facilitate achievement of quality and cost outcomes. The role of the Case Manager also includes working collaboratively with interdisciplinary staff internal and external to the organization and participating in quality improvement and evaluation processes related to the management of care.
