How You’ll Make an Impact
Create a positive, inclusive team environment where clinicians feel supported and motivated.
Lead and collaborate with an interdisciplinary care team to deliver coordinated behavioral health services in the home.
Serve as a trusted partner to clinicians, physicians, patients, and families — ensuring clear communication and shared goals.
Coach and support team members through regular feedback, problem-solving, and guidance.
Promote teamwork and accountability while ensuring high clinical standards and timely patient care.
Oversee plans of care with a focus on collaboration, continuity, and patient-centered outcomes.

What We’re Looking For
Associate’s Degree in Nursing (or related field) required
2+ years of home health or behavioral health experience as a Registered Nurse
At least 1 year of leadership or team support experience
Current state Registered Nurse license
Comfort using Workday, HomeCare HomeBase, and MS Office tools
The base salary for this position is $110,000 to $115,000 annually based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
This role may also be eligible for additional compensation, which could include performance-based bonuses, commissions, on-call pay, or other incentive earnings. These components are not guaranteed and depend on individual and/or team performance.
