What You’ll Do

• Conduct client assessments and develop individualized care plans
• Supervise and support caregivers to ensure safe, high-quality care
• Perform start-of-care and supervisory visits
• Monitor patient progress and adjust care as needed
• Educate clients, families, and caregivers
• Ensure compliance with clinical standards and regulations
• Maintain accurate, timely documentation
 

What We’re Looking For

• Active RN license (required)
• Experience in home care or working with elderly/disabled populations preferred
• Strong leadership, communication, and organizational skills
• Ability to manage multiple priorities in a field-based role
• Knowledge of home care regulations and compliance standards preferred
• Reliable personal transportation
