Job Summary:

To provide enhanced support for patients at risk of falls by assisting with mobility, toileting, and proactive engagement. This role is designed to strengthen fall prevention efforts and improve patient safety through trained, hands-on support.

Key Responsibilities:

• Monitor patients and intervene safely when they attempt to get out of bed or chairs without assistance.
• Assist patients with toileting needs, ensuring timely response and safe transfers.
• Reinforce fall prevention strategies, including the use of 5Ps, K Cards, and “No Pass” protocols.
• Maintain open communication with nursing staff regarding patient behavior, safety concerns, and care needs, while also participating in interdisciplinary huddles and safety briefings.
• Accurately document observations and interventions in accordance with hospital policy.

TECHNICAL CAPABILITIES:

• CLINICAL COMPETENCIES (Novice): Demonstrates competence with safety precautions and safe ambulation practice interventions, practical applications of minimal difficulty.
• PATIENT PARTNER PATIENT DOCUMENTATION (Novice): Independently works with clinical applications for patient documentation.
• ENVIRONMENT OF CARE (Novice): Demonstrates ability to set up and maintain patient rooms, supplies, and equipment. Possesses sufficient knowledge, training, and experience to be capable of successfully delivering environment of care services while requiring some support and instruction from others. • PLANNING & ORGANIZING (Novice): Demonstrates ability to anticipate basic needs and identify safety issues or concerns.  Prepares and presents clear and accurate status reports on patient care assignment.
