Job Duties:
  • Assists Veterans/Members with transfers, ambulation, eating, bathing, dressing, toileting, personal hygiene and other ADLs
  • Follows instructions to use simple procedures and devices to strengthen Veterans'/Members' limbs and maintain flexibility
  • Assists nursing staff with simple medical procedures such as suppositories, enemas, treatments, post-surgical care and post-mortem care
  • Assists in monitoring physical and mental status of Veterans/Members by utilizing appropriate medical equipment and techniques to take vital signs (temperature, pulse, respirations); obtain urine, sputum or stool samples; and track input/output
  • Responsible for medical record and worksheet documentation on all assigned Veterans/Members. Keeps records and prepares simple reports
  • Assists Veterans/Members with dining to include transporting to the dining room, informing them about menu choices available for the day, communicating dining staff about Veterans'/Members' choices and serves Veterans/Members their meals in courses, assisting them if necessary
  • Provides resident care and comfort by responding to patient requests (verbal or call light) for assistance timely in accordance with the Resident Bill of Rights
  • Maintains cleanliness of areas neat and sanitary by changing bedding, linens and other items
  • Reports to and assists licensed nurses in emergency situations rapidly and appropriately following established policies and procedure
  • Performs other related duties such as transporting Veterans/Members to appointments outside the facility and clerical duties (e.g., answering phones, filing, etc.) as needed
  • Actively involved in AMS processes such Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized Work.
  • Participates fully in the QAPI process, our Quality Assurance and Performance Improvement program mandate by CMS which incorporates AMS
Knowledge, Skills & Abilities (KSAs):

Knowledge of:

  • Principles, practices and techniques of para-professional nursing care, treatment and services appropriate to the work setting
  • State and agency laws and regulations governing para-professional nursing practices
  • Practices and procedures of using medical equipment in assigned duties
  • Practices and procedures established for the work environment
  • Practices and techniques applied in maintaining a safe, orderly, therapeutic and hygienic environment

Skills in:

  • Applying methods and techniques of para-professional nursing practices
  • Applying and interpreting State and agency laws and regulations governing para-professional nursing practices

Selective Preference(s):

One or more years of skilled nursing facility or long-term care experience preferred

Pre-Employment Requirements:
  • Current or temporary certification to practice as a Certified Nurse Assistant in the State of Arizona
  • Current Negative TB skin test (Mantoux skin test) within the last twelve months or a written statement from a physician, physician’s assistant or a registered nurse practitioner indicating freedom from Tuberculosis, if in the past has had a positive skin test or Tuberculosis
  • Must be able to obtain and retain a fingerprint clearance card issued by the Arizona Department of Public Safety
  • Current CPR card that was obtained by physical assessment

If this position requires driving or the use of a vehicle as an essential function of the job to conduct State business, then the following requirements apply: Driver’s License Requirements.

Keeps records and prepares simple reports Assists Veterans/Members with dining to include transporting to the dining room, informing them about menu choices available for the day, communicating dining staff about Veterans'/Members' choices and serves Veterans/Members their meals in courses, assisting them if necessary Provides resident care and comfort by responding to patient requests (verbal or call light) for assistance timely in accordance with the Resident Bill of Rights Maintains cleanliness of areas neat and sanitary by changing bedding, linens and other items Reports to and assists licensed nurses in emergency situations rapidly and appropriately following established policies and procedure Performs other related duties such as transporting Veterans/Members to appointments outside the facility and clerical duties (e.g., answering phones, filing, etc.) as needed Actively involved in AMS processes such Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized Work. Participates fully in the QAPI process, our Quality Assurance and Performance Improvement program mandate by CMS which incorporates AMS Knowledge, Skills & Abilities (KSAs): Knowledge of: Principles, practices and techniques of para-professional nursing care, treatment and services appropriate to the work setting State and agency laws and regulations governing para-professional nursing practices Practices and procedures of using medical equipment in assigned duties Practices and procedures established for the work environment Practices and techniques applied in maintaining a safe, orderly, therapeutic and hygienic environment Skills in: Applying methods and techniques of para-professional nursing practices Applying and interpreting State and agency laws and regulations governing para-professional nursing practices Using and maintaining medical equipment Applying methods and techniques in maintaining a safe, orderly, therapeutic and hygienic environment Recording, maintaining filing and retrieving medical records Verbal and written communication Ability to: Establish and maintain effective work relationships Learn, implement, and monitor Arizona Management System (AMS) methods Selective Preference(s): One or more years of experience working in a skilled nursing facility or long-term care environment preferred Pre-Employment Requirements: Current or temporary license to practice as a Certified Nursing Assistant in the State of Arizona Current CPR card that was obtained by physical assessment Current Negative TB skin test (Mantoux skin test) within the last twelve months or a written statement from a physician, physician’s assistant or a registered nurse practitioner indicating freedom from Tuberculosis, if in the past has had a positive skin test or Tuberculosis Must be able to obtain and retain a fingerprint clearance card issued by the Arizona Department of Public Safety Current CPR card that was obtained by physical assessment If this position requires driving or the use of a vehicle as an essential function of the job to conduct State business, then the following requirements apply: Driver’s License Requirements.
