Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) to conduct in-home supervisory visits for EPSDT and VA clients. This role focuses on oversight, assessment, and support rather than skilled nursing tasks.
The RN will manage a caseload of approximately 50–70 clients, ensuring required face-to-face visits every 60 days in the client’s home. This position requires strong organizational skills, independent work capability, and a commitment to high-quality, patient-centered care.

Cuba, NM – 6 clients
Santa Fe, NM – 33 clients
Las Vegas, NM – 29 clients
Key Responsibilities:
- Conduct in-home supervisory visits with EPSDT and VA clients every 60 days
- Ensure compliance with all federal, state, and organizational regulations and standards
- Assess client well-being, safety, and overall care needs
- Document visit findings accurately and in a timely manner
- Communicate concerns, observations, and recommendations to appropriate team members
- Collaborate with caregivers and interdisciplinary team members to ensure coordinated care
- Maintain complete, accurate, and up-to-date client records
- Provide education, guidance, and resources to clients and their families
- Adhere to all company policies, procedures, and confidentiality requirements
Qualifications:
- Current, active Registered Nurse (RN) license in New Mexico
- Minimum of 1 year of nursing experience (home health or community-based care preferred)
- Strong assessment, documentation, and organizational skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage a flexible schedule
- Demonstrated commitment to compassionate, patient-centered care
- Valid driver’s license and reliable transportation
- Must live in New Mexico
