***$10K Sign on bonus offered****

Hospice RN Case Manager position summary

Responsible for case managing a team of hospice patients. RN case manager will collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings. RN Case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms. Position is full time primarily Monday-Friday. No on call or weekends.

Minimum qualifications

  • Graduate of an approved nursing education program

  • Licensure as a Registered Nurse in the State of Georgia

  • 2 years experience as a professional nurse in acute care, home care or ER experience required

  • Home care experience preferred but not required

  • EPIC experience preferred
