Attends care planning and discharge planning review meetings. Provides patient/family education during stay and to prepare for discharge. Ensure that the rooms/beds assigned to new admissions, In-house transfer are ready to accept the patient. Conducts daily rounds with the Nurse Managers. Communicates and report relevant and significant patient related information or changes in condition to the Physician, Nurse Practitioner and Administrative Nurse Manager. Responds appropriately to patient care emergencies, provides guidance and direction to other caregivers in managing emergency events. Qualifications/Requirements: Current unencumbered RN license

MDS

MDS Coordinator

Coordinates and ensure completion of the state required Minimum Data set on all residents throughout the facility to include admissions, significant changes, quarterly and Medicare assessments in a timely manner. He/she identifies resident problems from the MDS and other assessments and develops the initial individual Care Plan for each resident. The MDS Coordinator reviews and optimizes the MDS Process to ensure appropriate services are rendered justifies facility reimbursement.Qualifications: Current Georgia Nursing Licensure, LPN or RN Experience in clinical and utilization experience in a Long Term Care or skilled facility Strong working knowledge of Medicare, Medicaid reimbursement guidelines, PPS and Rugs categories as it relates to the MDS process.

Responsibilities Include: Guides and directs and supervises the activities of the certified nursing assistants and LPN's. Reviews, modify as needed, patients’ care plans in collaboration with patient, family/designated representative and the interdisciplinary team. Director of Nursing

Assistant Director of Nursing (RN ADON SKILLED NURSING FACILITY)

The general purpose of the ADON is to manage administrative and functional areas or programs within the Nursing Services Department, and to assist the Director of Nursing in overall operation of the Department in accordance with facility policies, and Standards of nursing practices and government regulations, so as to maintain excellent care of all the Resident’s needs. ESSENTIAL DUTIES AND RESPONSIBILITIES: MANAGE AREAS OR PROGRAMS: Manage areas or programs to include carrying out, coordinating, or managing administrative functions and area or programs related to Nursing Services which may include departmental documentation or medical records, nursing supplies, ancillary supplies or services, facility QA, Care Plans, MDS, Infection Control Program, TILES, Restorative Care, Skin Care Program, Medicare, Special Restraint or Medication Programs, Light Duty or Safety Program, Staff Scheduling, Selection, Training, Orientation or related functions and such duties that may include supervision of other staff. SUPPORT DON: Work with or support Director of Nursing, Administrator and Consultants in planning all aspects of nursing service to include interacting with other disciplines and departments. May assist Director of Nursing in monitoring day to day operation of the nursing department, and to establish priorities and manage records, budgets or supplies. Assists or supports the Director of Nursing in managing nursing care to include monitoring quality of care and resident activity, assessment of resident’s status, administer medication and treatments, admissions or discharges; and to consult with family members and other Health Care professionals. STAND-IN: Fills in as acting Director of Nursing, charge nurse or other nursing positions as needed with limited or full authority as appropriate. MANAGE COMPLIANCE: Supports and assists the Director of Nursing to ensure compliance with State, Federal and Company QA standard, to include alerting management of potential non-compliance issues and preparation of correction plans. COMMUNITY RELATIONS: Develops positive relationships on behalf of the Company with Government Regulators, Families, Areas Health Care Community and the Community at large. RESIDENT’S RIGHTS: Ensure understanding of and compliance with all rules regarding residents’ rights. STAFF DEVELOPMENT: Supervise, conduct and participate in department and facility education activities and staff meetings. QUALIFICATIONS: Must be a Registered Nurse (RN) in good standing and currently licensed by the state. Must be CPR Certified. Completion of post-secondary education or otherwise able to read, write and follow oral and written directions at a level necessary to accomplish the job. Must be able to relate positively and favorably to residents and families and to work cooperatively with other associates. Must be capable of maintaining regular attendance. Must be capable of performing the Essential Job Functions of this job, with or without reasonable accommodation. Related experience at a level necessary to accomplish the job.
