As a Registered Nurse (RN), Home Infusion you will work outside the walls of a hospital setting in a growing specialized area of the nursing field. If you are spending 12 hours on your feet or rushing from patient to patient with no time to provide quality patient care, this would be the perfect opportunity to change your work environment. As a Home Infusion Nurse you will…

- Challenge your clinical nursing (RN) skills by receiving continuous training on leading edge technology and clinical requirements in specialty infusion. Certification and recertification expenses can be reimbursed for eligible RN’s such as Certified Registered Nurse of Infusion (CRNI®)!

- Provide a broad range of infusion therapies in our patients’ homes

- Make a difference by using your clinical IV experience and personal touch to assist our patients recover in the comfort of their own home.

- Provide one-on-one home infusion nursing care visits and patient education in accordance with the physician’s plan of treatment.

- Additional licensure may be required in multi state service areas

- Have excellent written and verbal customer service skills in order to interact with the patient, physician, pharmacist and other partners.

Schedule:

- This is a per-diem position working a flexible schedule of 10-20 hours/week.

- Hours are mostly during the day, but can go into the evening at times. Occassional weekend availability is welcomed.

- Telephonic on-call requirements of approximately five days/year. There are no in-person visits when on-call.

Coverage area:

- You will visit patients primarily in Eastern Suffolk County (North and South Coasts), but will need to have the flexibilty to visit patients in Western Suffolk occassionally.

- You will earn your hourly rate for both patient and drive time.

Required Qualifications:

- Registered Nurse with current license in NY

- Minimum one year nursing experience

- Strong IV insertion skills with a minimum of one year infusion/IV experience

- Have and maintain current CPR certification

- Possess a valid and current driver’s license and auto insurance

- Have experience with Microsoft Office, Excel, Outlook and Word to document and track patient care activity

- Participate in on call rotation as indicated by staffing and business needs

 

Preferred Qualifications:

- Home healthcare experience is a plus, but not required

- CRNI certification is a plus, but not required

Education:

RN Diploma, Associates or B.S. degree in nursing

The typical pay range for this role is:

$32.01 - $68.55

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Job Duties & Essential Functions: Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions and reports abnormalities to Medical Provider. Provide high quality, skilled infusion therapy. Highly skilled at starting intravenous lines, accessing ports and PICC Lines. Monitor patient status and medication being administered for duration of infusion. Demonstrates competency in answering phones, assembling charts, patient registration, making appointments, computer data entry and retrieval of reports. Must safely perform all patient care responsibilities in accordance with professional standards and the organization’s policies and procedures. Develop and organize a plan of care in consultation with treating physician, as indicated within the various subspecialties of the Department. Must effectively determine and set priorities to meet patient needs. Provide proper and accurate documentation of patient care in the patient medical record. Coordinate any services or accommodations necessary (e.g. sign language interpreter) to ensure effective communications between treating providers and patients who are hearing and/or visually impaired, and patients with language barriers. Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals. Administer medications, treatments, and contrast agents according to the specific orders of the ordering physician, and document in the patient record.
