Our Licensed Practical Nurses (LPN) provide high-quality, patient-centered care based on individualized needs, following established standards, policies, and procedures. Working under the direction and supervision of a Registered Nurse (RN), the LPN performs both independent and delegated nursing functions within the scope of practical nursing practice.
In this role, you will support patients and their families by delivering compassionate care, administering treatments, and collaborating with the care team to ensure positive patient outcomes. The LPN plays a key role in maintaining patient safety, supporting care coordination, and contributing to an exceptional patient experience.
WHAT YOU'LL DO
- Administers medications as directed by the RN and patient care plan, monitoring and documenting patient response.
- Provides safe, basic nursing care based on patient and family needs.
- Performs assigned nursing tasks in accordance with established policies and procedures.
- Communicates effectively with patients, families, and the healthcare team to support quality care.
- Completes accurate and timely documentation, including clinical notes, reports, and orders.
- Coordinates with pharmacies, insurance companies, and other providers to support patient care.
- Educates patients and families using appropriate teaching methods and materials.
- Maintains a safe environment and reports any concerns or incidents.
- Demonstrates professionalism and treats all individuals with respect.
- Handles calls and inquiries promptly and appropriately.
- Maintains patient confidentiality and follows HIPAA guidelines.
- LPNs will report any changes in patient condition to the RN immediately
- Adherence with state regulations governing licensed nursing practice
- Performs other duties as assigned.
WHAT YOU HAVE
- Is a graduate of an accredited practical nursing program
- Has passed the National Council Licensure Examination (NCLEX)
- Is currently licensed in good standing as an LPN through the State Board of Nursing
- Complies with accepted professional standards and principles
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order, or other reliable means of transportation
- Possesses and maintains CPR certification
- Has presented a pre-employment physician’s health clearance including negative TB skin test and/or CXR and other tests as required by organization policy
- Has satisfactory references from nursing school, previous (or current) employers and/or professional peers
Preferred Education and Experience:
- Associate’s degree with two years of related work experience
TRAVEL REQUIREMENTS:
- Daily local travel required by car to perform patient visits, attend in-person meetings, continued education, etc. Occasional travel required to corporate offices outside assigned region
