If you are located in or within commutable driving distance to the Southeast Houston, TX area, you will have the flexibility to work a Hybrid/Field schedule as you take on some tough challenges.

This is a Field Based position with a Home-Based office.

Primary Responsibilities
- Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
- Support members with condition education, medication reviews, and connections to resources such as Provider Services, Private Duty Nursing, DME, and supplies
- Assist members with the transition from a care facility back to their home
- This role involves approximately 75% travel, and you'll need to be comfortable dealing with a full range of in-home and facility-based environments
- Most of the time you'll work autonomously, so the ability to stay focused and motivated is essential
- This function is for UHC C&S Only (with the exception of the Community Health Worker role)
- Positions in this function address efforts focused on medical, behavioral, and social determinants of health factors that improve the well-being of individuals, families, and communities
- Generally, work is self-directed and not prescribed
- Works with less structured, more complex issues

Required Qualifications
- Associate's degree (or higher)
- Current unrestricted RN license
- 3+ years of clinical experience in a hospital, acute care, home health/hospice, direct care, or case management
- Intermediate level of computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
- Valid driver's license, access to reliable transportation and the ability to travel up to 75% in an assigned region to visit Medicaid members in their homes and/or other settings
- Reside in Houston, TX area
- Case management experience including Certification in Case Management
- Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs
- Experience in Home & Community based on Long-Term Care services delivery
- Home care/field-based case management
- Bilingual skills
- Reside in the SE Houston, TX area
